目的:探讨眼镜蛇咬伤后局部软组织炎症反应及血管内皮因子(VEGF)分泌的变化。方法:将20例因眼镜蛇咬伤致局部组织溃疡的患者分为治疗组10例、对照组10例。两组均常规清创换药。治疗组常规创面清创后,以复方四黄液外敷伤口肿胀处;对照组以50%硫酸镁外敷伤口肿胀处。溃疡发生后第1d、第7d分别在创缘处切取大小2mm×2mm大小的组织,检测 VEGF、巨噬细胞、中性多核白细胞(PMN)、淋巴细胞。7d 后两组患者按照创面情况选用不同手术方式治疗、修复创面。结果:两组患者组织中VEGF、巨噬细胞、PMN、淋巴细胞检测结果表明,溃疡发生后第1d两组无明显差异(P>0.05),溃疡发生后第7d,与对照组比较,治疗组创面巨噬细胞计数、VEGF显著增加(P<0.05),而其PMN及淋巴细胞计数则明显降低(P<0.01)。治疗组创面修复愈合平均时间短于对照组。结论:中西医结合治疗可通过调节巨噬细胞、淋巴细胞趋化以及升高血管内皮生长因子水平而降低炎症反应,改善局部血流,促进伤口愈合。

목적:탐토안경사교상후국부연조직염증반응급혈관내피인자(VEGF)분비적변화。방법:장20례인안경사교상치국부조직궤양적환자분위치료조10례、대조조10례。량조균상규청창환약。치료조상규창면청창후,이복방사황액외부상구종창처;대조조이50%류산미외부상구종창처。궤양발생후제1d、제7d분별재창연처절취대소2mm×2mm대소적조직,검측 VEGF、거서세포、중성다핵백세포(PMN)、림파세포。7d 후량조환자안조창면정황선용불동수술방식치료、수복창면。결과:량조환자조직중VEGF、거서세포、PMN、림파세포검측결과표명,궤양발생후제1d량조무명현차이(P>0.05),궤양발생후제7d,여대조조비교,치료조창면거서세포계수、VEGF현저증가(P<0.05),이기PMN급림파세포계수칙명현강저(P<0.01)。치료조창면수복유합평균시간단우대조조。결론:중서의결합치료가통과조절거서세포、림파세포추화이급승고혈관내피생장인자수평이강저염증반응,개선국부혈류,촉진상구유합。
Objective: To observe the effects of FUFANGSIHUANGYE (FFSHY) on inflammatory cell chemotaxis and VEGF secretion in ulcerated wound by cobrasnake bite. Methods :20 ulcerated wound by cobrasnake bite cases were divided into treatment group (n=10) and control group (n=10). The external application of FFSHY was used in the treatment group and 50% magnesium sulphate was used in the control group. The tissue of wound edge were taken to detect the content of VEGF, Macrophages, PMN, lymphocytes on the 1st, 7st. According to the condition of wounds,patients were chosen different surgical methods 7 days later. Results: There was no significant difference about the content of VEGF, Macrophages, PMN, lymphocytes (P>0.05) on the 1st.After 7 days, the content of VEGF, Macrophages in treatment group had a higher expression level than control group. On the contrary, the content of PMN, lymphocytes was lower. The average time of wound healing in treatment group was shorter than control group. Conclusion: The method of combination traditional Chinese and western medicine can reduce the inflammation, incressing local blood flow, promoting the healing process.
