目的:观察血液分析仪白细胞相关指标,如白细胞数(WBC)、未成熟粒细胞(IG%、IG#)、幼稚细胞(OTHER%、OTH-ER#)、白细胞分类散点图,在白血病初步筛查和鉴别中的应用价值。方法选取血液分析仪白细胞检测异常初诊患者,记录其外周血液中WBC、IG%、IG#、幼稚细胞数、白细胞分类散点图等资料。另外,抽取患者骨髓进行细胞形态学和免疫组化分析进行白血病确诊和分类。根据最终诊断将55例患者分成急性髓细胞性白血病(AML)组30例,慢性粒细胞性白血病(CML)组10例,急性淋巴细胞白血病(ALL)组15例,统计分析各组白细胞相关指标的差异。结果 AML患者幼稚细胞百分比、IG%明显增高,WBC正常或中度升高;CML患者WBC、IG%、IG#明显增高,但幼稚细胞增高不明显;ALL患者WBC明显增高、幼稚细胞百分比轻度增高、IG%、IG#一般不增高。白细胞分类散点图都有一定的异常,各组患者均有特征性的散点图异常。结论通过白细胞相关指标的综合分析,为白血病筛查和初步分类提供理论依据,给临床进一步检查指明方向。

목적:관찰혈액분석의백세포상관지표,여백세포수(WBC)、미성숙립세포(IG%、IG#)、유치세포(OTHER%、OTH-ER#)、백세포분류산점도,재백혈병초보사사화감별중적응용개치。방법선취혈액분석의백세포검측이상초진환자,기록기외주혈액중WBC、IG%、IG#、유치세포수、백세포분류산점도등자료。령외,추취환자골수진행세포형태학화면역조화분석진행백혈병학진화분류。근거최종진단장55례환자분성급성수세포성백혈병(AML)조30례,만성립세포성백혈병(CML)조10례,급성림파세포백혈병(ALL)조15례,통계분석각조백세포상관지표적차이。결과 AML환자유치세포백분비、IG%명현증고,WBC정상혹중도승고;CML환자WBC、IG%、IG#명현증고,단유치세포증고불명현;ALL환자WBC명현증고、유치세포백분비경도증고、IG%、IG#일반불증고。백세포분류산점도도유일정적이상,각조환자균유특정성적산점도이상。결론통과백세포상관지표적종합분석,위백혈병사사화초보분류제공이론의거,급림상진일보검사지명방향。
Objective To analyze the change of white blood cell related indicators obtained from hematology analyzer, such as white blood cell count (WBC), immature granulocytes (IG%, IG#), immature cells (OTHER%, OTHER#), DIFF scatter diagram, and explore their application value in the primary screening and identification of leukemia. Methods Newly diagnosed patients with abnormal white blood cell related indicators performed by Sysmex XE-2100 hematology analyzer were enrolled, and their pe-ripheral blood WBC, IG%, IG#, immature cells, leukocyte scatter diagram and other informations were recorded. In addition, the final diagnosis and classification of leukemia would be determined by cytomorphology and immunohistochemical analysis with bone marrow cells. According to the final diagnosis, 55 cases were divided into acute myeloid leukemia (AML) group (n=30), chronic myelogenous leukemia (CML) group (n=10), and acute lymphoblastic leukemia (ALL) group (n=15), the difference of white blood cells related indicators in each group was performed by statistical analysis. Results From the AML group, the percentage of imma-ture cells and IG% were significantly higher, and the WBC was normal or moderately elevated; From the CML group, WBC, IG%and IG# significantly increased, while the increase of immature cells was not obvious; From the ALL group, WBC remarkably in-creased, the percentage of immature cells slightly increased, while IG%, IG # generally was not increased. Moreover, abnormal WBC scatter diagram were observed in all three groups with different features. Conclusion A comprehensive analysis of white blood cell related indicators provides a theoretical basis for the screening and preliminary classification of leukemia, which would guide the clinicians for further examinations.
