目的：比较股骨近端锁定钢板（LPFP）与标准伽玛－Ⅲ型带锁髓内钉（Gamma-Ⅲ）治疗外侧壁危险型股骨粗隆间骨折的临床疗效。方法对37例外侧壁危险型股骨粗隆间骨折患者根据内固定方式分别采用LPFP和Gamma-Ⅲ治疗。其中LPFP组21例，男性7例，女性14例，年龄60～85岁，平均（75．8±6．9）岁。Gamma-Ⅲ组16例，男性9例，女性7例，年龄62～83岁，平均（71．5±7．3）岁。比较两组患者受伤至手术时间、术中失血量、手术时间及术后6个月Harris评分情况。结果37例患者均获得随访，平均随访10．6个月（6～12个月），全部患者骨折愈合，恢复外伤前行走功能。两组患者受伤至手术时间、术中失血量、手术时间及术后6个月Harris评分情况比较差异均无统计学意义（P＞0．05）。结论 LPFP以及Gamma-Ⅲ均能有效治疗外侧壁危险型股骨粗隆间骨折。
목적：비교고골근단쇄정강판（LPFP）여표준가마－Ⅲ형대쇄수내정（Gamma-Ⅲ）치료외측벽위험형고골조륭간골절적림상료효。방법대37예외측벽위험형고골조륭간골절환자근거내고정방식분별채용LPFP화Gamma-Ⅲ치료。기중LPFP조21례，남성7례，녀성14례，년령60～85세，평균（75．8±6．9）세。Gamma-Ⅲ조16례，남성9례，녀성7례，년령62～83세，평균（71．5±7．3）세。비교량조환자수상지수술시간、술중실혈량、수술시간급술후6개월Harris평분정황。결과37례환자균획득수방，평균수방10．6개월（6～12개월），전부환자골절유합，회복외상전행주공능。량조환자수상지수술시간、술중실혈량、수술시간급술후6개월Harris평분정황비교차이균무통계학의의（P＞0．05）。결론 LPFP이급Gamma-Ⅲ균능유효치료외측벽위험형고골조륭간골절。
Objective To compare the curative effect between locking proximal femoral plate(LPFP)and gamma-Ⅲ standard inter-locking intramedullary nail(Gamma-Ⅲ)for the treatment of patients with intertrochanteric fracture with risk external wall. Methods From February 2012 to April 2014,37 patients with intertrochanteric fracture with risk external wall were treated by LPFP or Gamma-Ⅲaccording to different kinds of internal fixations. There were 21 cases in LPFP,including 7 males and 14 females with an average age of 75. 8 ± 6. 9 (ranging from 60 to 85)years old;while 16 cases in Gamma-Ⅲ,including 9 males and 7 females with an average age of 71. 5 ± 7. 3(ranging from 62 to 83)years old. Blood loss,operation time and Harris score after 6 months′following up were observed and compared. Results All patients were followed up for 6 to 12 (means 10. 6)months,and all patients obtained fracture healing and recovered walking ability as be-fore injury. There were no significant differences in blood loss,operation time and Harris score after 6 months'following up(P>0. 05). Con-clusion Both of LPFP and Gamma-Ⅲ in treating patients with intertrochanteric fracture with risk external wall can achieve good curative effect.
