目的：观察盐酸羟考酮缓释片（奥施康定）联合吗啡片在中、重度癌痛治疗中的镇痛效果及不良反应。方法选择112例中重度癌痛患者，随机分为吗啡组50例（给予吗啡即释片镇痛）和联合组62例（给予羟考酮缓释片联合吗啡片镇痛）。对两组的镇痛疗效、起效时间及不良反应进行观察及评估。结果与吗啡组比较，联合组的滴定剂量降低，镇痛起效时间明显缩短，差异具有统计学意义（P<0.05）；联合组便秘及恶心呕吐的发生率均明显低于吗啡组，差异具有统计学意义（P<0.05）。结论羟考酮缓释片联合吗啡片滴定方案治疗中、重度癌痛起效快、镇痛效果好，副反应轻，是治疗癌痛较好的选择之一。
목적：관찰염산간고동완석편（오시강정）연합마배편재중、중도암통치료중적진통효과급불량반응。방법선택112례중중도암통환자，수궤분위마배조50례（급여마배즉석편진통）화연합조62례（급여간고동완석편연합마배편진통）。대량조적진통료효、기효시간급불량반응진행관찰급평고。결과여마배조비교，연합조적적정제량강저，진통기효시간명현축단，차이구유통계학의의（P<0.05）；연합조편비급악심구토적발생솔균명현저우마배조，차이구유통계학의의（P<0.05）。결론간고동완석편연합마배편적정방안치료중、중도암통기효쾌、진통효과호，부반응경，시치료암통교호적선택지일。
Objective To observe the analgesic effects and main side effects of controlled-release oxycodone (oxycontin) and oral immediate-release morphine in the treatment of moderate and severe cancer pain. Methods A total of 112 patients with severe can-cer pain were divided into morphine group in which immediate-release morphine is involved with 50 cases, and combined group in which oxycodone sustained-release tablets combined with morphine tablets with 62 cases. Therapeutic effects and time to onset and adverse reaction was compared between the two groups. Results Compared with the morphine group, the titration dose of the com-bined group decreased, and the analgesic onset time was shortened obviously, with statistic difference (P<0.05). The incidence of constipation and nausea and vomiting in the combination group were significantly lower than that of morphine group, also with sig-nificant difference (P<0.05). Conclusion The dosage titration therapy with combination of controlled-release oxycodone and oral immediate-release morphine worked quickly, safely and effectively on moderate and severe cancer pain, and had comparatively slight side effects. It is a good choice for cancer pain.
