目的：观察醒脑开窍汤治疗急性脑梗死的临床疗效。方法：86例急性脑梗死患者按照随机数字表法分为对照组和治疗组各43例。两组均给予西医常规治疗；对照组患者在此基础上给予奥扎格雷钠注射液静脉滴注；治疗组则在对照组基础上给予醒脑开窍汤，1剂·d－1，7 d为1个疗程，连续治疗2个疗程。观察两组临床有效率，治疗前后两组患者肢体麻木、眩晕、乏力、语言障碍、口舌歪斜等临床症状的改善情况，两组证候积分的变化情况；对患者的神经功能缺损程度、日常生活能力、肢体运动功能等进行评分；观察治疗前后患者大脑中动脉血流的变化情况。结果：治疗组的有效率为90．7％，明显高于对照组的有效率69．8％，两组有效率比较，差异有统计学意义（P＜0．05）；两组患者经治疗后，两组患者肢体麻木、眩晕、乏力语言障碍、口舌歪斜等症状与治疗前相比均有所改善，证候积分与治疗前比有所下降，下降幅度以治疗组尤为明显，差异有统计学意义（P＜0．05）；两组患者的神经功能、日常活动能力、肢体运动功能经治疗后明显提高，神经功能缺损评分有所降低，日常生活能力及运动功能评分均升高明显，治疗组患者的变化幅度尤为突出，差异有统计学意义（P＜0．05）；经治疗后患者双侧大脑中动脉血流收缩期峰值流速、收缩期峰值流速、血流速度均有明显升高，以治疗组的升高幅度更大，差异有统计学意义（P＜0．05）。结论：醒脑开窍汤能有效改善急性脑梗死患者的临床症状，提高患者的神经功能、日常活动能力、肢体运动功能，利于患者双侧大脑的动脉血液循环。
목적：관찰성뇌개규탕치료급성뇌경사적림상료효。방법：86례급성뇌경사환자안조수궤수자표법분위대조조화치료조각43례。량조균급여서의상규치료；대조조환자재차기출상급여오찰격뢰납주사액정맥적주；치료조칙재대조조기출상급여성뇌개규탕，1제·d－1，7 d위1개료정，련속치료2개료정。관찰량조림상유효솔，치료전후량조환자지체마목、현훈、핍력、어언장애、구설왜사등림상증상적개선정황，량조증후적분적변화정황；대환자적신경공능결손정도、일상생활능력、지체운동공능등진행평분；관찰치료전후환자대뇌중동맥혈류적변화정황。결과：치료조적유효솔위90．7％，명현고우대조조적유효솔69．8％，량조유효솔비교，차이유통계학의의（P＜0．05）；량조환자경치료후，량조환자지체마목、현훈、핍력어언장애、구설왜사등증상여치료전상비균유소개선，증후적분여치료전비유소하강，하강폭도이치료조우위명현，차이유통계학의의（P＜0．05）；량조환자적신경공능、일상활동능력、지체운동공능경치료후명현제고，신경공능결손평분유소강저，일상생활능력급운동공능평분균승고명현，치료조환자적변화폭도우위돌출，차이유통계학의의（P＜0．05）；경치료후환자쌍측대뇌중동맥혈류수축기봉치류속、수축기봉치류속、혈류속도균유명현승고，이치료조적승고폭도경대，차이유통계학의의（P＜0．05）。결론：성뇌개규탕능유효개선급성뇌경사환자적림상증상，제고환자적신경공능、일상활동능력、지체운동공능，리우환자쌍측대뇌적동맥혈액순배。
Objective:To investigate the clinical efficacy of Xingnao Kaiqiao Decoction in treating acute cerebral infarction.Methods:86 acute cerebral infarction cases were randomly divided into control group (n=43)and treatment group (n=43).The control group was given Sodium Ozagrel Injection and the treatment group was given Xingnao Kaiqiao Decoction based on conventional western medicine foundation treatment,lagent a day,7days for a course,two courses for treatment.We observed the improvement of clinical symptoms, mainly including numbness,dizziness,weak language barriers,facial paralysis,and so on.Compared the change of symptom score in pre-therapy and post-treatment.We evaluated the degree of nervous functional defects,ability to daily life and limb locomotor function.We observed the change of middle cerebral artery blood flow before and after treatment,and calculated the effective rate of the two groups. Results:The efficiency for patients in the treatment group was 90.7%,which was higher than that of the control group 69.8%,the difference is significant (P<0.05).The main clinical symptoms of the patients in two groups,such as numbness,dizziness,weak lan-guage barriers,facial paralysis were improved clearly,and the symptom scores went down clearly,the degree of change in the treatment group was more pronounced,the difference was significant (P<0.05).The patients′nervous functional defects,ability to daily life and limb locomotor function were improved clearly,and the neurologic impairment score,viability index and motor function score increased significantly,the degree of change in the treatment group was more pronounced,the difference is significant (P<0.05 ).The middle cerebral artery flow velocity systole period peak flow rate and speed of blood flow in two groups after treatment were higher than that of before treatment.The degree of change in the treatment group was more pronounced,the difference is significant (P<0.05 ).Conclu-sion:Treating acute cerebral infarction patients with Xingnao Kaiqiao Decoction and sodium ozagrel injection can improve the clinical symptoms,improve the patients′nervous functional defects,ability to daily life and limb locomotor function.It has the prominent func-tion of restoring consciousness,resuscitation and anti-infection,which is good for the brain artery blood circulation.It is an effective scheme of combining traditional Chinese with western medicine for treating nervous system disease,which has exact curative effect,is worthy of further research and development.
