目的:探讨慢性阻塞性肺疾病(简称慢阻肺)患者血清脂联素与其营养状况、炎症因子的相关性。方法慢阻肺患者58例为病例组,健康人群30例为健康对照组,检测两组的血清脂联素、肿瘤坏死因子-α水平,及营养指数评分系统,评估受试者的营养状况,对比两组受试者MNI评分结果。结果病例组营养不良的发生率为46.55%,显著高于健康对照组(P<0.05);病例组的%IBW为(86.27±9.82)%,上臂肌围为(23.41±3.58)cm,血清脂联素水平为(8.04±3.91)μg/L,均显著低于健康对照组,而肿瘤坏死因子-α水平为(20.89±2.49)ng/L,显著高于健康对照组(均P<0.05)。结论慢阻肺患者血清脂联素水平和肿瘤坏死因子水平呈负相关关系,血清脂联素参与患者营养状况和炎症反应的调节。

목적:탐토만성조새성폐질병(간칭만조폐)환자혈청지련소여기영양상황、염증인자적상관성。방법만조폐환자58례위병례조,건강인군30례위건강대조조,검측량조적혈청지련소、종류배사인자-α수평,급영양지수평분계통,평고수시자적영양상황,대비량조수시자MNI평분결과。결과병례조영양불량적발생솔위46.55%,현저고우건강대조조(P<0.05);병례조적%IBW위(86.27±9.82)%,상비기위위(23.41±3.58)cm,혈청지련소수평위(8.04±3.91)μg/L,균현저저우건강대조조,이종류배사인자-α수평위(20.89±2.49)ng/L,현저고우건강대조조(균P<0.05)。결론만조폐환자혈청지련소수평화종류배사인자수평정부상관관계,혈청지련소삼여환자영양상황화염증반응적조절。
Objective To investigate the correlation of serum adiponectin to nutritional status and inflamma-tion factors of COPD patients. Methods 58 patients with chronic obstructive pulmonary disease were selected as the observation group, and 30 healthy subjects were selected as the control group. The serum adiponectin, tumor necrosis factor alpha, nutritional status and nutritional index were compared. Results The incidence of malnutrition was 46. 55%, which was significantly higher than that in the healthy control group (P<0. 05). In the observation group, the value of IBW% was (86. 27 ± 9. 82)%, the arm muscle circumference was (23. 41 ± 3. 58) cm, and the serum adiponectin level was ( 8. 04 ± 3. 91 ) μg/L, which were all significantly lower than those in the healthy control group. The level of tumor necrosis factor alpha was (20. 89 ± 2. 49) ng/L in the observation group, which were sig-nificantly higher than that in the healthy control group ( P<0. 05 ) . Conclusion The level of serum adiponectin is negatively correlated with tumor necrosis factor level, which may participate in the regulation of nutritional status and inflammatory response in patients with COPD.
