目的:探讨S100A4蛋白在肺癌组织中的表达,并探讨其临床意义。方法搜集45例肺鳞癌、50例腺癌、23例小细胞肺癌病例,调查其分化程度、肿瘤大小、淋巴结转移及临床分期等参数;采用免疫组化SP法分别检测各型肺癌组织及癌旁组织中S100A4蛋白的表达,应用Image-Pro Plus 6.0软件进行图像分析。结果所有肺鳞癌、腺癌标本中S100A4蛋白的表达均强于相应的癌旁组织,组间比较差异有统计学意义( P<0.01)。小细胞肺癌细胞内未检测到S100A4的表达,但在其间质血管中检测到S100A4的表达。在鳞癌患者中的S100A4蛋白表达中,Ⅰ期IOD值为19012.12±4856.98,Ⅱ期为27654.40±5365.86,Ⅲ期为39001.38±6435.97,IV 期为48753.00±7727.42(P <0.05),而在腺癌中,Ⅰ期为22343.11±5156.18,Ⅱ期为31942.04±4915.36,Ⅲ期为44519.23±5435.07,Ⅳ期为51060.53±10757.74(P<0.05)。鳞癌和腺癌标本中S100A4蛋白的表达有淋巴结转移组明显高于无淋巴结转移组(P<0.05),有远处转移组明显高于无远处转移组(P<0.05)。肺鳞癌、腺癌标本中S100A4蛋白的表达与患者的性别、年龄、肿瘤大小、肿瘤分化程度无关(P>0.05)。结论 S100A4蛋白在非小细胞肺癌的增殖及转移中起重要作用,检测S100A4蛋白有助于非小细胞肺癌预后的判断;同时它可能在小细胞肺癌的侵袭转移中也起到一定作用。

목적:탐토S100A4단백재폐암조직중적표체,병탐토기림상의의。방법수집45례폐린암、50례선암、23례소세포폐암병례,조사기분화정도、종류대소、림파결전이급림상분기등삼수;채용면역조화SP법분별검측각형폐암조직급암방조직중S100A4단백적표체,응용Image-Pro Plus 6.0연건진행도상분석。결과소유폐린암、선암표본중S100A4단백적표체균강우상응적암방조직,조간비교차이유통계학의의( P<0.01)。소세포폐암세포내미검측도S100A4적표체,단재기간질혈관중검측도S100A4적표체。재린암환자중적S100A4단백표체중,Ⅰ기IOD치위19012.12±4856.98,Ⅱ기위27654.40±5365.86,Ⅲ기위39001.38±6435.97,IV 기위48753.00±7727.42(P <0.05),이재선암중,Ⅰ기위22343.11±5156.18,Ⅱ기위31942.04±4915.36,Ⅲ기위44519.23±5435.07,Ⅳ기위51060.53±10757.74(P<0.05)。린암화선암표본중S100A4단백적표체유림파결전이조명현고우무림파결전이조(P<0.05),유원처전이조명현고우무원처전이조(P<0.05)。폐린암、선암표본중S100A4단백적표체여환자적성별、년령、종류대소、종류분화정도무관(P>0.05)。결론 S100A4단백재비소세포폐암적증식급전이중기중요작용,검측S100A4단백유조우비소세포폐암예후적판단;동시타가능재소세포폐암적침습전이중야기도일정작용。
Objective To investigate the expression of S100A4 protein in lung cancer tissues and further to discuss its clinical significance in lung cancer. Methods The study selected 45 cases of lung squamous cell carcino-ma, 50 cases of adenocarcinoma and 23 cases of small cell lung carcinoma. The expression of S100A4 protein in vari-ous types of lung cancer and adjacent non-malignant tissues were detected by immunohistochemistry method respec-tively, and quantitative analysis of the difference among them were done by Image-Pro Plus 6. 0 software system. Re-sults The expression of S100A4 protein in all cases of lung squamous cell carcinomas and adenocarcinoma was sig-nificantly higher than that in the adjacent non-malignant tissues and bronchial mucosa inflammatory lesions (P<0. 01). The expression of S100A4 protein was not detected in small cell lung carcinoma, but it was detected in stromal vascular. For the expression of S100A4 protein, the IOD values at stages Ⅰ,Ⅱ,Ⅲ and Ⅳ patients with squamous cell carcinoma were 19012. 12 ± 4856. 98, 27654. 40 ± 5365. 86, 39001. 38 ± 6435. 97 and 48753. 00 ± 7727.42 respectively (P < 0.05), and they were 22343.11 ± 5156.18, 31942.04 ± 4915.36, 44519.23 ± 54315. 07 and 51060. 53 ± 10757. 74 respectively at stage Ⅰ, Ⅱ, Ⅲ, Ⅳ patients with adenocarcinoma ( P <0. 05). The S100A4 protein expression of lung squamous cell carcinomas and adenocarcinoma was significantly higher in the group with lymphatic metastasis than in the group without lymphatic metastasis (P<0. 05). The S100A4 pro-tein expression of the lung squamous cell carcinomas and adenocarcinoma was significantly higher in the group with distant metastasis than in the group without distant metastasis (P<0. 05). The S100A4 protein expression of lung squamous cell carcinomas and adenocarcinoma was not related with gender, age, tumor size or pathological grade ( P>0. 05). Conclusion The S100A4 protein play an important role in the proliferation and metastasis of non small cell lung cancer, and the detection of S100A4 protein is helpful to judge the prognosis of non small cell lung cancer. At the same time, it may play a certain role in invasion and metastasis of small cell lung cancer.
