目的:观察振动排痰机治疗慢性阻塞性肺疾病急性加重期( AECOPD)的临床疗效。方法128例AECOPD患者,随机分为观察组和对照组,各64例,两组均给予抗炎、解痉平喘、止咳化痰、雾化吸入、持续低流量吸氧或无创呼吸机辅助呼吸等常规治疗,对照组加用人工扣背排痰法,观察组加用体外振动排痰机行机械叩振排痰法。结果①观察组较对照组HR、RR、PaCO2、pH值下降更明显,PaO2、SaO2上升更明显,住院天数缩短,抗生素使用天数缩短,24小时排痰量增加,差异均具有统计学意义(P<0.01)。②观察组患者的气急、胸闷、呼吸困难改善,呼吸功能较对照组明显改善。③观察组湿啰音消失率84.4%(54/64)与对照组67.2%(43/64)相比差异有统计学意义(P<0.05)。结论体外振动排痰机治疗AECOPD患者具有显著的排痰效果,能改善患者的血气与肺功能状态,具有操作简单、容易掌握、痛苦小、缩短疗程,住院时间短等优点,是治疗AECOPD的一种安全有效的方法。

목적:관찰진동배담궤치료만성조새성폐질병급성가중기( AECOPD)적림상료효。방법128례AECOPD환자,수궤분위관찰조화대조조,각64례,량조균급여항염、해경평천、지해화담、무화흡입、지속저류량흡양혹무창호흡궤보조호흡등상규치료,대조조가용인공구배배담법,관찰조가용체외진동배담궤행궤계고진배담법。결과①관찰조교대조조HR、RR、PaCO2、pH치하강경명현,PaO2、SaO2상승경명현,주원천수축단,항생소사용천수축단,24소시배담량증가,차이균구유통계학의의(P<0.01)。②관찰조환자적기급、흉민、호흡곤난개선,호흡공능교대조조명현개선。③관찰조습라음소실솔84.4%(54/64)여대조조67.2%(43/64)상비차이유통계학의의(P<0.05)。결론체외진동배담궤치료AECOPD환자구유현저적배담효과,능개선환자적혈기여폐공능상태,구유조작간단、용역장악、통고소、축단료정,주원시간단등우점,시치료AECOPD적일충안전유효적방법。
Objective To observe the curative effect of vibration row phlegm machine in the treatment of pa-tients with AECOPD. Methods 128 AECOPD patients were randomly divided into the observation group and the control group, 64 cases in each group. All patients were given conventional treatment of anti inflammation, spasm and asthma, cough and phlegm, inhalation, persistent low flow oxygen inhalation or noninvasive ventilator assisted breathing. The control group was treated with manual button back expectoration, and the observation group was given external vibration row phlegm machine mechanical percussion vibration sputum elimination method. Results ①The value of HR, RR, PaCO2 and pH decreased more significantly in the observation group than in the control group, and the level of PaO2 and SaO2 increased more significantly in the observation group than in the control group. The dura-tion of hospital stay and the days of antibiotic use were lower in the observation group than in the control group, and 24 hours sputum volume was higher in the observation group than in the control group (P<0. 01). ②The improve-ment of shortness of breath, chest tightness, dyspnea and respiratory function was more pronounced in the observation group than in the control group. ③ The rate of moist rales disappearance was 84. 4% (54/64) in the observation group and 67. 2% in the control group (43/64) (P<0. 05). Conclusion Extrasomatic vibration sputum elimina-tion machine can improve the blood gas and pulmonary function of AECOPD patients, with the advantages of simple operation, easy to master and short duration of hospital stay.
