目的:研究根管治疗后前磨牙Ⅱ类洞型使用不同桩系统结合直接树脂充填后对牙齿抗折强度以及劈裂形式的影响。方法选择选择2012年6—9月正畸拔除上颌前磨牙40颗,随机分为5组,每组8颗。 A 组:不进行任何处理;其余组牙齿全部进行根管治疗,B 组直接复合树脂充填修复,C 组钛螺纹桩+树脂充填,D 组玻璃纤维桩+树脂充填,E 组石英纤维桩+树脂充填。修复后进行抗折强度检测。结果 E 组与 A 组载荷力值相比无明显差异(P >0.05),C、D 组均显著低于 A 组(P <0.05),C、D、E 组显著高于 B 组(P <0.05)。不可修复劈裂形式并没有明显减少。结论此洞型使用根管内桩系统结合直接树脂粘接修复,提高了牙齿的抗力,石英纤维桩修复能够达到接近健康牙齿的抗力;可修复的劈裂形式数量没有明显优势。

목적:연구근관치료후전마아Ⅱ류동형사용불동장계통결합직접수지충전후대아치항절강도이급벽렬형식적영향。방법선택선택2012년6—9월정기발제상합전마아40과,수궤분위5조,매조8과。 A 조:불진행임하처리;기여조아치전부진행근관치료,B 조직접복합수지충전수복,C 조태라문장+수지충전,D 조파리섬유장+수지충전,E 조석영섬유장+수지충전。수복후진행항절강도검측。결과 E 조여 A 조재하력치상비무명현차이(P >0.05),C、D 조균현저저우 A 조(P <0.05),C、D、E 조현저고우 B 조(P <0.05)。불가수복벽렬형식병몰유명현감소。결론차동형사용근관내장계통결합직접수지점접수복,제고료아치적항력,석영섬유장수복능구체도접근건강아치적항력;가수복적벽렬형식수량몰유명현우세。
Objective To investigate the effects of different post systems combined with direct composite restora-tions on the rupture strength and fracture mode of premolar teeth with class II cavities after root canal therapy. Methods A total of 40 single-rooted orthodontic premolars in human upper jaw from June to September 2012 were randomly divided into 5 groups (n =8 for each group). Groups A was untreated and the other 4 groups were endodontically treated. Group B was repaired with composite resin filling only; group C was repaired with composite resin and a titanium screw; group D was repaired with composite resin and fiberglass-reinforced composite post; group E was repaired with composite resin and fiber-reinforced composite post. The rupture strength was detected in each group after repair. Results There was no significant difference in fracture load values between group A and E (P >0. 05), but the values in group C and D were significantly lower than that in group A (P <0. 05), and the values in group C, D and E were significantly higher than that in group B (P <0. 05). There was no statistically significant decrease in fracture modes in all the groups. Conclusion The different post systems combined with direct composite restorations in repair of premolar teeth with class II cavities can improve the fracture resistance of teeth, and composite resin and fiber-reinforced composite post therapy may achieve the similar load-bearing capability of healthy premolars, but no advantage has been found in number of repaired fracture modes.
