目的:探讨肾癌患者CT引导下经皮射频消融(RFA )治疗后相关感染因素,并分析预防对策。方法对2010年1月-2014年1月478例行C T引导下经皮射频消融治疗肾癌患者进行临床回顾性研究,探讨术后切口感染的危险因素,采用SPSS 17.0软件进行统计分析。结果478例肾癌患者中发生医院感染18例,感染率为3.77%;Kap‐lan‐M eier单因素分析显示,转移性肾癌、邻近空腔脏器、肿瘤多发、肿瘤直径>5 mm与RFA术后肾内感染存在相关性(P<0.05);多因素 logistic回归统计显示,继发性肿瘤、邻近空腔脏器肿瘤、多发肿瘤是术后患者出现感染的独立高危因素(P<0.05)。结论继发性肿瘤、邻近空腔脏器肿瘤、多发肿瘤等是CT 引导下经皮RFA后出现肾内感染的高危因素。

목적:탐토신암환자CT인도하경피사빈소융(RFA )치료후상관감염인소,병분석예방대책。방법대2010년1월-2014년1월478례행C T인도하경피사빈소융치료신암환자진행림상회고성연구,탐토술후절구감염적위험인소,채용SPSS 17.0연건진행통계분석。결과478례신암환자중발생의원감염18례,감염솔위3.77%;Kap‐lan‐M eier단인소분석현시,전이성신암、린근공강장기、종류다발、종류직경>5 mm여RFA술후신내감염존재상관성(P<0.05);다인소 logistic회귀통계현시,계발성종류、린근공강장기종류、다발종류시술후환자출현감염적독립고위인소(P<0.05)。결론계발성종류、린근공강장기종류、다발종류등시CT 인도하경피RFA후출현신내감염적고위인소。
OBJECTIVE To discuss the relevant infection factors of renal cancer after the treatment of PRFA guided by CT and to analyze prevention measures .METHODS A total of 478 cases of patients with renal cancer and a‐dopted the treatment of PRFA which was guided by CT from Jan .2010 to Jan .2014 in our hospital were retro‐spectively studied .Risk factors of incision infections after operation were discussed .All data were analyzed by SPSS 17 .0 software .RESULTS Among the 478 cases of renal cancer patients ,18 cases had suffered from nosoco‐mial infections with the infection rate of 3 .77% .Kap‐lan‐Meier single factor analysis indicated that metastatic re‐nal cell carcinoma ,neighboring hollow organs ,multiple tumors and diameter of tumor more than 5 mm were relat‐ed to the infections within kidney after RFA operation (P<0 .05) .Multi‐factor logistic regression analysis indica‐ted that secondary tumor ,tumor neighboring hollow organs and multiple tumors were the independent and high risk factors of infections after operation (P<0 .05) .CONCLUSION Secondary tumor ,tumor neighboring hollow organs and multiple tumors were the high risk factors of infections within kidney after PRFA guided by CT .
