目的:探讨血清超敏C反应蛋白(hs-CRP)、胱抑素C(Cys C)和尿β2-微球蛋白(β2-MG)联合检测在妊娠糖尿病(GDM)肾病诊断中的临床应用价值。方法:89例GDM患者和110例健康孕妇为研究对象。在清晨抽取3ml空腹静脉血,经离心后得到血清。同时收集24h尿液。采用全自动生化分析仪检测血清尿酸、尿素、肌酐、hs-CRP和Cys C;用免疫比浊法检测尿β2-MG。结果:GDM患者血清hs-CRP、Cys C和尿β2-MG水平分别为(2.96±1.38)mg/L、(1.35±0.42)mg/L和(1.75±0.49)mg/L,高于对照组(P<0.01);而两组血清尿素、肌酐的差异无统计学意义(P>0.05);GDM患者组的尿酸高于对照组(P<0.05)。结论:GDM患者血hs-CRP、Cys C和尿β2-MG水平的升高与GDM患者肾损伤的发生密切相关,联合检测可以提高GDM早期肾损伤诊断的准确性。

Objective:To investigate the clinical value of the combined examination of serum high-sensitive C-reactive protein (hs-CRP), cystatin C (Cys C), andβ2 urine microglobulin (β2-MG) for diagnosing renal damage of patients with gestational diabetes mellitus. Methods:Totally 89 gestational diabetes mellitus (GDM) patients and 110 healthy pregnant women in our hospital were analyzed. The serum hs-CRP, Cys C, urea, creatinine, uric acid and urineβ2-MG concentrations were measured in each group. Results:Serum hs-CRP, Cys C and urineβ2-MG of pa-tients of GDM group were obviously higher than those in the control group ( P<0.01), while the differences of serum urea, creatinine between the two groups showed no statistical significance ( P >0.05). Serum uric acid of GDM group was high er than this in the control group ( P<0.05). Conclusion:Cys C, hs-CRP, andβ2-MG are sensitive in-dicators for early diagnosing renal damage of patients with GDM. Combined examination can improve the accuracy of the early diagnosis of GDM.
