目的:调查高龄患者呼吸机相关性肺炎患者病原菌分布及耐药性特点,为临床诊治提供依据。方法回顾性分析2010年1月-2014年7月102例高龄患者呼吸机相关性肺炎的临床资料,调查感染病原菌分布及耐药性,采用SPSS17.0处理数据。结果102例高龄患者呼吸机相关性肺炎送检的标本中共分离鉴定病原菌187株,革兰阴性菌116株占62.0%,革兰阳性菌58株占31.0%,真菌13株占7.0%;革兰阴性菌对青霉素和头孢菌素具有较高的耐药性,耐药率均>60.0%;革兰阳性菌对糖肽类抗菌药物如万古霉素、利奈唑胺和替考拉宁等敏感率为100.0%。结论高龄患者呼吸机相关性肺炎感染病原菌主要为革兰阴性菌,其耐药现象较为严重,临床应加强病原菌监测,指导临床合理使用抗菌药物。

목적:조사고령환자호흡궤상관성폐염환자병원균분포급내약성특점,위림상진치제공의거。방법회고성분석2010년1월-2014년7월102례고령환자호흡궤상관성폐염적림상자료,조사감염병원균분포급내약성,채용SPSS17.0처리수거。결과102례고령환자호흡궤상관성폐염송검적표본중공분리감정병원균187주,혁란음성균116주점62.0%,혁란양성균58주점31.0%,진균13주점7.0%;혁란음성균대청매소화두포균소구유교고적내약성,내약솔균>60.0%;혁란양성균대당태류항균약물여만고매소、리내서알화체고랍저등민감솔위100.0%。결론고령환자호흡궤상관성폐염감염병원균주요위혁란음성균,기내약현상교위엄중,림상응가강병원균감측,지도림상합리사용항균약물。
OBJECTIVE To investigate the infection of pathogenic bacteria and drug resistance in elderly patients with ventilator associated pneumonia ,and provide basis for clinical treatment .METHODS From Jan ,2010 to Jul , 2014 ,a retrospective analysis was carried out on 102 elderly patients with ventilator associated pneumonia .The distribution and drug resistance of pathogenic bacteria were analyzed .The software SPSS17 .0 was used for data processing .RESULTS Totally 187 strains were identified from the 102 patients ,containing 116 strains of gram‐negative bacteria (62 .0% ) ,58 strains of gram‐positive bacteria (31 .0% ) and 13 strains of fungi (7 .0% ) .The gram‐negative bacteria showed high drug resistance to penicillin and cephalosporin (the drug resistance rate was o‐ver 60 .0% ) .The gram‐positive bacteria were sensitive to glycopeptide antibiotics (such as vancomycin ,linezolid and teicoplanin) ,and the sensitive rate was 100 .0% .CONCLUSION Most of pathogenic bacteria in elderly pa‐tients with ventilator associated pneumonia were belonged to gram‐negative bacteria and with serious drug resist‐ance .It is necessary to strengthen the monitoring of pathogenic bacteria for clinical rational use of antibacterial drugs .
