目的:探讨经伤椎单侧椎弓根固定及植骨治疗胸腰椎骨折的临床疗效。方法对23例胸腰椎骨折患者应用椎弓根钉棒系统治疗,经伤椎一侧椎弓根螺钉固定,经另一侧椎弓根通道植骨。 X 线片评价术前、术后1周及末次随访时的伤椎前缘高度、后凸 Cobb 角。结果患者均获随访,时间8~24(10±2)个月,无内固定物失效,胸腰椎高度和后凸矫正度无明显丢失,骨折愈合良好。结论经伤椎单侧椎弓根固定及植骨治疗胸腰椎骨折可以减少内固定失败和矫正度的丢失,操作简单,临床效果满意。

목적:탐토경상추단측추궁근고정급식골치료흉요추골절적림상료효。방법대23례흉요추골절환자응용추궁근정봉계통치료,경상추일측추궁근라정고정,경령일측추궁근통도식골。 X 선편평개술전、술후1주급말차수방시적상추전연고도、후철 Cobb 각。결과환자균획수방,시간8~24(10±2)개월,무내고정물실효,흉요추고도화후철교정도무명현주실,골절유합량호。결론경상추단측추궁근고정급식골치료흉요추골절가이감소내고정실패화교정도적주실,조작간단,림상효과만의。
Objective To investigate the clinical effect of treatment of thoracolumbar burst fracture by pedicle screw fixation through single pedicle passage of injured vertebra combined with bone graft through the opposite side. Meth-ods 23 cases of thoracolumbar fracture were treated by pedicle screw fixation, the injured vertebra was fixed with pedicle screw at one side, and the other side got transpedicular bone grafting simultaneously. All data of the patients including the anterior marginal height of the injured vertebra and the Cobb′s angle were evaluated in preoperation, one week after operation and the last follow-up. Results All cases were followed up for 8 ~ 24(10 ± 2) months, the internal fixation instrument failure and lost of correction of the anterior marginal height of the injured vertebra and the Cobb′s angle were not observed obviously, all the patients got fracture healing. Conclusions By placing pedicle screw in one side combined with transpedicular bone grafting in the other side at the fractured vertebra is an effective and simple method in the treatment of thoracolumbar fractures, because it is helpful in preventing failure of fixation, correction loss and vertebra deformity.
