目的：应用超声监测药物流产前孕囊大小及流产后子宫有无残留物，分析影响药物流产效果的相关因素，以降低药物流产失败后手术干预率。方法选取600例药物流产病例，流产前超声估测孕囊≤49 d，服用米非司酮配伍米索前列醇，流产一周后应用超声判断流产效果，根据流产效果分为药物流产失败后手术组和流产成功对照组，分析比对相关影响因素。结果怀孕次数越多、流产次数越多、孕囊直径越大的病例，药物流产失败率越高；有分娩史比无分娩史、子宫前后位比水平位的病例，药物流产失败率更高。结论孕产次、流产史、孕囊大小、有无分娩史、子宫位置均是影响药物流产效果的相关因素，有无剖宫产史对药物流产影响不大。超声检查在药物流产前后均发挥重要作用。
목적：응용초성감측약물유산전잉낭대소급유산후자궁유무잔류물，분석영향약물유산효과적상관인소，이강저약물유산실패후수술간예솔。방법선취600례약물유산병례，유산전초성고측잉낭≤49 d，복용미비사동배오미색전렬순，유산일주후응용초성판단유산효과，근거유산효과분위약물유산실패후수술조화유산성공대조조，분석비대상관영향인소。결과부잉차수월다、유산차수월다、잉낭직경월대적병례，약물유산실패솔월고；유분면사비무분면사、자궁전후위비수평위적병례，약물유산실패솔경고。결론잉산차、유산사、잉낭대소、유무분면사、자궁위치균시영향약물유산효과적상관인소，유무부궁산사대약물유산영향불대。초성검사재약물유산전후균발휘중요작용。
Objective To measure the gestational sac size and to check whether there is a residue in the womb after abortion by ultrasound,then we could find the related factors influencing drug abortion and reduce the rate of operation after the failure of drug abortion.Methods Select 600 cases of drug abortion. Before the abortion,their gestational sac size were estimated to equal to or less than 49 days by ultrasound. These women oral mifepristone and misoprostol,and we judge the effect of abortion by ultrasound after a week. 600 cases were divided into two groups:the operation group after drug abortion failure and the drug abortion group. Results The study found that if the greater number of pregnancies and abortion,the bigger of gestational sac diameter,so the higher failure rate of drug abortion. The delivery history cases have higher abortion failure rate than non delivery history cases. Anterior and posterior of the uterus than the horizontal position have higher failure rate of drug abortion.Conclusion The number of pregnancy and abortion,the size of gestational sac,the delivery history and uterine position are all related factors on drug abortion. Cesarean section history has little impact on drug abortion. Ultrasound play an important role in both before and after drug abortion.
