目的探讨2型糖尿病(T2DM )患者尿mindin与糖尿病肾病(DN )的关系。方法采用ELISA法测定90例T2DM患者(A组)和32例健康体检者(B组)尿mindin水平。依据尿白蛋白/尿肌酐相对值(ACR)水平,A组再分为无蛋白尿(A1组)、微量蛋白尿(A2组)和大量蛋白尿(A3组)三个亚组。检测并分析相关指标。结果 A组患者尿mindin相对值(MCR=尿mindin/尿肌酐)与尿微量蛋白、糖化血红蛋白、收缩压、病程、血肌酐和24‐h尿蛋白呈正相关( P<0.05或P<0.01),与肾小球滤过率呈负相关(r=-0.395,P<0.01)。慢性肾脏病分期为1、2、3期患者的尿MCR递增:3期(16例)>2期(45例)>1期(24例)(P<0.01),且与ACR变化规律类似。结论尿mindin可能与T2DM患者肾损伤密切有关;联合检测尿mindin和尿微量白蛋白有助于发现早期 DN。

목적탐토2형당뇨병(T2DM )환자뇨mindin여당뇨병신병(DN )적관계。방법채용ELISA법측정90례T2DM환자(A조)화32례건강체검자(B조)뇨mindin수평。의거뇨백단백/뇨기항상대치(ACR)수평,A조재분위무단백뇨(A1조)、미량단백뇨(A2조)화대량단백뇨(A3조)삼개아조。검측병분석상관지표。결과 A조환자뇨mindin상대치(MCR=뇨mindin/뇨기항)여뇨미량단백、당화혈홍단백、수축압、병정、혈기항화24‐h뇨단백정정상관( P<0.05혹P<0.01),여신소구려과솔정부상관(r=-0.395,P<0.01)。만성신장병분기위1、2、3기환자적뇨MCR체증:3기(16례)>2기(45례)>1기(24례)(P<0.01),차여ACR변화규률유사。결론뇨mindin가능여T2DM환자신손상밀절유관;연합검측뇨mindin화뇨미량백단백유조우발현조기 DN。
Objective To investigate the changes and significance of urinary mindin protein in type 2 diabetic patients with nephropathy .Methods Mindin protein in urine was determined using ELISA in 90 patients with type 2 diabetes mellitus(T2DM ,group A) and 32 healthy people(group B) . On the basis of urine albumin/creatinine ratio (ACR) ,group A was assigned into three subgroups of A1(ACR<3 mg/mmol ,29 cases) ,A2(3 mg/mmol≤ACR≤30 mg/mmol ,29 cases) and A3(ACR>30 mg/mmol ,32 cases) .The related clinical data and biochemical indexes were collected and analyzed . Results The urinary midin/creatinine ratio(MCR) was positively correlated with urine trace protein , glycated hemoglobin ,duration ,systolic blood pressure ,serum creatinine ,and urine 24‐h protein ( P<0 .05 or P<0 .01 ) ,but negatively correlated with glomerular filtration rate ( r= -0 .395 , P<0 .01) .The urinary MCR of group A was in an order of chronic kidney disease (CKD) stage 3 (16 cases)> CKD stage 2(45 cases)> CKD stage 1(24 cases) .The tendency of MCR change was similar to that of ACR .Conclusion Urinary mindin protein may be closely related with early renal injury in T2DM patients .Combined detection of urinary mindin and trace protein is helpful in finding early diabetic nephropathy .
