目的：探讨高能超声聚焦刀（ HIFU）联合动脉栓塞治疗单发性小肝癌的价值。方法经病理证实的小肝癌患者70例随机分为2组，每组35例，A组行传统治疗方案及动脉栓塞，B组行肿瘤部位动脉栓塞的同时行HIFU。结果 A组治疗后1个月复查肿瘤缩小者16例，完全坏死液化0例，稳定无明显缩小3例，肿瘤增大或扩散转移16例；B组治疗30 min后所有肿瘤治疗部位回声增强，彩色多普勒血流减少，临床治疗1个月后复查肿瘤缩小者24例，完全坏死液化3例，稳定无明显缩小2例，肿瘤增大或扩散转移6例。结论 HIFU联合动脉栓塞治疗小肝癌安全有效。
목적：탐토고능초성취초도（ HIFU）연합동맥전새치료단발성소간암적개치。방법경병리증실적소간암환자70례수궤분위2조，매조35례，A조행전통치료방안급동맥전새，B조행종류부위동맥전새적동시행HIFU。결과 A조치료후1개월복사종류축소자16례，완전배사액화0례，은정무명현축소3례，종류증대혹확산전이16례；B조치료30 min후소유종류치료부위회성증강，채색다보륵혈류감소，림상치료1개월후복사종류축소자24례，완전배사액화3례，은정무명현축소2례，종류증대혹확산전이6례。결론 HIFU연합동맥전새치료소간암안전유효。
Objective To study the high-energy ultrasonic focusing knife( HIFU)combined with arterial embol-ism in the treatment of small hepatocellular carcinoma. Methods Seventy patients with small hepatocellular carci-noma were randomly divided into two groups,each group of 35 patients,the 35 patients in the group A received tra-ditional treatment and arterial embolism,the other 35 patients in the group B received artery embolism and HIFU treatment. Results In the group A,shrinking tumor was observed in the 16 patients,completely liquefaction necro-sis in the 0 patient,stability in the 3 patients,tumor increases or diffusion transfer in the 16 patients,in the 1 months after the treatment. In the group B,echogenicity was in all the tumor site in the 30 minutes after the treatment,blood flow was decreased in the color Doppler,shrinking tumor in the 24 patients,completely liquefaction necrosis in the 3 patients,stability in the 2 patients,tumor increases or diffusion transfer in the 6 patients,in the 1 months after the treatment. Conclusion HIFU combined with arterial embolism is safe and effective for small hepatocellular carcino-ma.
