目的：测量肱骨近端骨折患者的肱骨头纵轴长度，并评价AO肱骨近端内固定锁定系统（ proximal humerus internal locking osteosynthesis system，PHILOS）结构螺钉的置入情况。方法：回顾性分析北京大学人民医院2007年1月至2014年2月使用PHILOS接骨板治疗的肱骨近端骨折患者117例，根据性别分组分别测量肱骨头纵轴长度，根据术后肩关节正位X线片评价结构螺钉的置入情况并计算结构螺钉的置入率。结果：入选患者117例，男性40例，女性77例；男性患者肱骨头纵轴长度为（47．64±3．44） mm，支撑螺钉置入率为52．5％；女性患者肱骨头纵轴长度平均为（42．46±3．21） mm，支撑螺钉置入率为32．5％，男性组与女性组在肱骨头纵轴长度和结构螺钉置入率方面差异具有统计学意义（P＜0．05）。结论：PHILOS接骨板用于肱骨近端骨折治疗，结构螺钉置入率低，特别是女性，可以在中国人解剖学数据基础上予以改良。
目的：測量肱骨近耑骨摺患者的肱骨頭縱軸長度，併評價AO肱骨近耑內固定鎖定繫統（ proximal humerus internal locking osteosynthesis system，PHILOS）結構螺釘的置入情況。方法：迴顧性分析北京大學人民醫院2007年1月至2014年2月使用PHILOS接骨闆治療的肱骨近耑骨摺患者117例，根據性彆分組分彆測量肱骨頭縱軸長度，根據術後肩關節正位X線片評價結構螺釘的置入情況併計算結構螺釘的置入率。結果：入選患者117例，男性40例，女性77例；男性患者肱骨頭縱軸長度為（47．64±3．44） mm，支撐螺釘置入率為52．5％；女性患者肱骨頭縱軸長度平均為（42．46±3．21） mm，支撐螺釘置入率為32．5％，男性組與女性組在肱骨頭縱軸長度和結構螺釘置入率方麵差異具有統計學意義（P＜0．05）。結論：PHILOS接骨闆用于肱骨近耑骨摺治療，結構螺釘置入率低，特彆是女性，可以在中國人解剖學數據基礎上予以改良。
목적：측량굉골근단골절환자적굉골두종축장도，병평개AO굉골근단내고정쇄정계통（ proximal humerus internal locking osteosynthesis system，PHILOS）결구라정적치입정황。방법：회고성분석북경대학인민의원2007년1월지2014년2월사용PHILOS접골판치료적굉골근단골절환자117례，근거성별분조분별측량굉골두종축장도，근거술후견관절정위X선편평개결구라정적치입정황병계산결구라정적치입솔。결과：입선환자117례，남성40례，녀성77례；남성환자굉골두종축장도위（47．64±3．44） mm，지탱라정치입솔위52．5％；녀성환자굉골두종축장도평균위（42．46±3．21） mm，지탱라정치입솔위32．5％，남성조여녀성조재굉골두종축장도화결구라정치입솔방면차이구유통계학의의（P＜0．05）。결론：PHILOS접골판용우굉골근단골절치료，결구라정치입솔저，특별시녀성，가이재중국인해부학수거기출상여이개량。
Objective:To measure vertical axis length of humerus head in patients with proximal hu-meral fractures and to evaluate the use of structure screws in proximal humerus internal locking osteosyn-thesis system ( PHILOS) .Methods:From January 2007 to February 2014,117 patients with proximal hu-merus fracture were treated by PHILOS plate.Preoperatively, all the patients tookanteroposterior X-ray of shoulder, and the vertical axis length of humerus head were measured, and the use of structure screws in PHILOS plate was observed.Results: There were 40 male and 77 female patients; The vertical axis length of humerus head in male patients was (47.64 ±3.44) mm, and the rate of structure screw use was 52.5%;The vertical axis length of humerus head in female patients was (42.46 ±3.21) mm, and the rate of structure screw use was 32 .5%.The vertical axis length of humerus head and the rate of structure screw use had significant difference in the male and female groups (P<0.05).Conclusion:The rate of structure screw use was low in the treatment of proximal humerus fracture by PHILOS plate, especially for women.We should improve the design of plate according to the anatomical characteristics of Chinese.
