目的：探讨颈内静脉穿刺置管与颈外静脉留置针在急诊患者中的应用优势。方法：114例分别使用颈内静脉穿刺置管和颈外静脉留置套管针进行输液、输血等的患者，对两种置管方法操作时间、留置时间、成功例数、导管异位例数、主要并发症情况进行观察。结果：颈外静脉留置针组的操作时间与留置时间明显低于颈内静脉穿刺组（P<0.01），颈外静脉留置针组的成功率高于穿刺组，导管异位率低于颈内静脉穿刺组（P<0.05），颈外静脉留置针组的并发症发生率显著低于颈内静脉穿刺组（P<0.05）。结论：颈外静脉留置针与颈内静脉穿刺置管相比，具有操作时间短、操作简单、成功率较高、导管异位率和并发症低的特点。颈外静脉留置针可为静脉穿刺困难患者建立血管通路，对患者生命的维持起到重要作用。

목적：탐토경내정맥천자치관여경외정맥류치침재급진환자중적응용우세。방법：114례분별사용경내정맥천자치관화경외정맥류치투관침진행수액、수혈등적환자，대량충치관방법조작시간、류치시간、성공례수、도관이위례수、주요병발증정황진행관찰。결과：경외정맥류치침조적조작시간여류치시간명현저우경내정맥천자조（P<0.01），경외정맥류치침조적성공솔고우천자조，도관이위솔저우경내정맥천자조（P<0.05），경외정맥류치침조적병발증발생솔현저저우경내정맥천자조（P<0.05）。결론：경외정맥류치침여경내정맥천자치관상비，구유조작시간단、조작간단、성공솔교고、도관이위솔화병발증저적특점。경외정맥류치침가위정맥천자곤난환자건립혈관통로，대환자생명적유지기도중요작용。
Objective:To explore the application advantage of internal jugular vein puncture and indwelling needle of external jugular vein in emergency patients. Method:114 patients were respectively given internal jugular vein puncture and indwelling needle of external jugular vein for infusion and blood transfusion., successful cases, ectopic tube cases and main complications of the two methods were observed. Result:The operation time and indwelling time of the indwelling needle of external jugular vein group was significantly shorter than that of the internal jugular vein puncture group (P<0.01). The successful rate of the indwelling time of the indwelling needle of external jugular vein group was higher than that of the internal jugular vein puncture group. The incidence rate of ectopic tube of indwelling needle of external jugular vein group was lower than that of the internal jugular vein puncture group (P<0.05). The incidence rate of complications of indwelling needle of external jugular vein group was lower than that of the internal jugular vein puncture group (P<0.05). Conclusion:Compared with internal jugular vein puncture, indwelling needle of external jugular vein has the characteristics of short operation time, convenient to operate, high successful rate, low incidence rate of ectopic tube and complication. Indwelling needle of external jugular vein can build up the vascular access for patients with difficulty in venipuncture. It has important effect on sustaining life of patients.
