目的：探讨低剂量右美托咪定药物在老年高血压患者硬膜外麻醉中的应用效果。方法：2013年4月-2014年4月收治实施手术的老年高血压患者104例，依据随机数据表法分成甲组与乙组，各52例，其中对甲组予以右美托咪定，对乙组予以氯化钠溶液，并对其血压变化情况进行观察与比较。结果：比较两组患者的收缩压、舒张压、心率、血氧饱和度在给药后5 min，麻醉后0.5 h、麻醉后2 h的变化，其中甲组的收缩压、舒张压、心率显著降低，差异有统计学意义(P＜0.05)，而血氧饱和度无明显变化，不具统计学意义(P＞0.05)。结论：对需行硬膜外麻醉的老年高血压患者，在围麻醉期间给予低剂量的右美托咪定药物，可发挥一定的镇静功效，缓解高血压患者的紧张情绪，继而帮助患者安全、顺利地度过围麻醉期，值得在临床上大力推广与运用。
목적：탐토저제량우미탁미정약물재노년고혈압환자경막외마취중적응용효과。방법：2013년4월-2014년4월수치실시수술적노년고혈압환자104례，의거수궤수거표법분성갑조여을조，각52례，기중대갑조여이우미탁미정，대을조여이록화납용액，병대기혈압변화정황진행관찰여비교。결과：비교량조환자적수축압、서장압、심솔、혈양포화도재급약후5 min，마취후0.5 h、마취후2 h적변화，기중갑조적수축압、서장압、심솔현저강저，차이유통계학의의(P＜0.05)，이혈양포화도무명현변화，불구통계학의의(P＞0.05)。결론：대수행경막외마취적노년고혈압환자，재위마취기간급여저제량적우미탁미정약물，가발휘일정적진정공효，완해고혈압환자적긴장정서，계이방조환자안전、순리지도과위마취기，치득재림상상대력추엄여운용。
Objective:To explore the application of low dose dexmedetomidine epidural anesthesia in senile hypertension patients. Methods:104 cases of elderly patients with hypertension were selected from April 2013 to April 2014.According to random table method,they were divided into the A group and the B group with 52 cases in each group,A group was given dexmedetomidine,and B group was given sodium chloride solution,we compared the change of blood pressure.Results:We compared systolic pressure, diastolic pressure,heart rate,blood oxygen saturation at 5 min after administration,0.5 hours after anesthesia,2 hours after anesthesia of the two groups,in A group,systolic pressure,diastolic pressure,heart rate was significantly reduced,the difference was statistically significant(P<0.05),and oxygen saturation had no obvious change,there was no significant difference(P>0.05). Conclusion:For elderly patients with hypertension on the need for epidural anesthesia,at the stage of anesthesia,giving low doses of dexmedetomidine can play a sedative effect.It can relieve tension in hypertension patients,so as to help the patients successfully through the perianesthesive period,and it is worthy of promotion and application.
