背景肺高压(pulmonary hypertension,PH)患者行非心脏手术期间并发症发病率和病死率显著增高,围术期的麻醉处理是麻醉医师面临的挑战之一. 目的综述近年PH及相关麻醉管理的研究文献,为麻醉医师的临床工作提供理论参考. 内容阐述PH的定义及诊断,围术期并发症和死亡的预测因素,术前准备与评估,麻醉方式、麻醉管理以及严重并发症的处理. 趋向随着对PH研究的深入,非心脏手术围术期麻醉管理会得到进一步优化,从而减少并发症和死亡的发生率.

Background Morbidity and mortality is significantly increased in patients with pulmonary hypertension (PH) undergoing noncardiac surgeries.Perioperative treatment of anesthesia is one of challenges for anesthesiologists.Objective The article summarizes recent studies about the anesthesia in PH and anesthetic management in order to provide theoretical foundation of clinical practice for anesthesiologists.Content This review expounds information of regarding the definition of PH,diagnosis,risk factors for morbidity and mortality in noncardiac surgeries,preoperative preparation and assessment,approach and management,as well as treatment of severe complications.Trend With the further researches in PH,better perioperative management will decrease the morbidity and mortality for noncardiac surgery patients.
