目的：观察氯吡格雷用于脑血管狭窄支架置入术后患者治疗的疗效及安全性。方法将入院时诊断为脑血管狭窄患者30例，随机分为研究组和对照组，研究组于支架置入术前一天给以氯吡格雷300mg，顿服，术后150mg，每天1次，连用2周，然后改为75mg，每天1次，连用6个月；对照组于手术前一天开始给以氯吡格雷75mg，每天1次，连用6个月。结果研究组从手术当日开始血小板聚集率持续下降，与治疗前相比，差异有统计学意义；对照组手术当日与基线水平相当，术后第3天血小板聚集率明显下降，与手术当日相比差异有统计学意义，术后7天、术后1月与研究组相比差异不显著，术后6月与研究组相比差异明显，有统计学意义。结论术前给以氯吡格雷顿服，对脑血管狭窄介入术后患者的疗效好，且安全。
목적：관찰록필격뢰용우뇌혈관협착지가치입술후환자치료적료효급안전성。방법장입원시진단위뇌혈관협착환자30례，수궤분위연구조화대조조，연구조우지가치입술전일천급이록필격뢰300mg，돈복，술후150mg，매천1차，련용2주，연후개위75mg，매천1차，련용6개월；대조조우수술전일천개시급이록필격뢰75mg，매천1차，련용6개월。결과연구조종수술당일개시혈소판취집솔지속하강，여치료전상비，차이유통계학의의；대조조수술당일여기선수평상당，술후제3천혈소판취집솔명현하강，여수술당일상비차이유통계학의의，술후7천、술후1월여연구조상비차이불현저，술후6월여연구조상비차이명현，유통계학의의。결론술전급이록필격뢰돈복，대뇌혈관협착개입술후환자적료효호，차안전。
Objectives To observe curative effect and safety in cerebrovascular stent implantation patients with clopidogrel. Methods 30 patients with cerebrovascular stenosis were randomly divided into two groups.Research group: Clopidogrel 300mg loading dose before cerebrovascular stent implantation, clopidogrel 150mg in the second day and the follow two weeks, then clopi-dogrel 75mg QD in the followed six months.The patients in control group started to take clopidogrel 75mg QD before cerebrovascular stent implantation and in the followed six months.Results The platelet inhibition of research group was declined from the day of cerebrovascular stent implantation, there was significant difference compared with prior treatment.It was similar between the day of cerebrovascular stent implantation and base line in control group,the third day platelet inhibition was dramatic declined,there was significant difference compared with the day of cerebrovascular stent implantation;It was similar in the seventh day and one month between two groups,there was significant difference between two groups at postoperation six month.Conclusions b300mg loading dose before cerebrovascular stent implantation is effective and safe.
