目的 探讨产褥期产妇性激素水平及阴道微生态的变化.方法 选择产褥期复查的足月分娩的初产妇308例,根据分娩方式分为阴道分娩组(n=125)和剖宫产组(n=183).比较两组产妇产后阴道微生态及性激素水平的变化情况.结果 两组产后复查时间及血性恶露时间比较无明显差异(P>0.05).两组阴道pH值、pH值异常率、BV阳性率无明显差异(P>0.05).pH值正常组血性恶露持续时间、BV阳性率均明显低于pH值异常组(P<0.01).剖宫产组PRL、P、T均明显低于阴道分娩组(P<0.05).两组阴道优势菌均发生了明显变化.结论 产褥期女性性激素水平及阴道微生态均发生了明显改变,因此应指导产妇尽早下地活动,缩短血性恶露持续的时间,必要时给予乳杆菌类制剂,以促进产褥期女性阴道微生态的恢复.

Objective To investigate the changes of sex hormone level and vaginal micro ecolog ical of puerperal women.Methods Atotal of 308 women after full-term delivery who were reviewed in prenatal were selected and divided into vaginal delivery group(n =125) and cesarean section group(n =183) according to mode of delivery.The changes of micro ecological environment and sex hormone level of puerperal women were compared.Results There was no significant difference in terms ofpost partum lochia rubra time and postpartum review time between the two groups(P > 0.05).The pH value,rate of pH abnormal value,and positive rate of BV had no significant differences between the two group (P > 0.05).The lochia rubra duration,positive rate of BV in normal pH value group were significantly lower than those in pH abnormal group(P < 0.01).The PRL,P,T in cesarean section group were significantly lower than those in vaginal delivery group (P < 0.05).The vaginal predominant bacteria both changed significantly.Conclusions Postpartum female sex hormone levels and vaginal micro ecological were changed significantly,so it is suggested that guiding maternal early ambulation,shorten the lochia rubra duration,and give lactobacillus if necessary,in order to promote recovery of puerperal femal vagina micro ecological.
