目的 在立体定向放射治疗中,对不同部位肿瘤患者摆位误差进行测量与分析,探讨其影响因素及控制方法.方法 选取54例肿瘤患者,其中17例脑转移患者,27例转移性或原发性肺癌患者,10例转移性肝癌患者,行立体定向放射治疗,治疗前行CBCT扫描,并结合扫描结果确定靶区,对靶区中心在X、Y、Z轴上的摆位误差进行统计分析.结果 头部摆位误差较小,其误差平均值<1mm,而胸腹部患者的摆位误差则相对较大,平均值<3 mm,但头部和胸腹部的摆位误差均控制在精确放射治疗的要求范围内;对X轴、Y轴以及Z轴三个方向的误差范围和平均值来看,胸腹部放射治疗的患者比头部放射治疗的患者摆位重复性准确率低.结论 胸腹部摆位极易受患者的呼吸、体质量变化以及膀胱充盈度等因素的影响而发生变化,通过对影响因素进行综合分析,采取相应的控制措施,提高放疗重复摆位精度.
目的 在立體定嚮放射治療中,對不同部位腫瘤患者襬位誤差進行測量與分析,探討其影響因素及控製方法.方法 選取54例腫瘤患者,其中17例腦轉移患者,27例轉移性或原髮性肺癌患者,10例轉移性肝癌患者,行立體定嚮放射治療,治療前行CBCT掃描,併結閤掃描結果確定靶區,對靶區中心在X、Y、Z軸上的襬位誤差進行統計分析.結果 頭部襬位誤差較小,其誤差平均值<1mm,而胸腹部患者的襬位誤差則相對較大,平均值<3 mm,但頭部和胸腹部的襬位誤差均控製在精確放射治療的要求範圍內;對X軸、Y軸以及Z軸三箇方嚮的誤差範圍和平均值來看,胸腹部放射治療的患者比頭部放射治療的患者襬位重複性準確率低.結論 胸腹部襬位極易受患者的呼吸、體質量變化以及膀胱充盈度等因素的影響而髮生變化,通過對影響因素進行綜閤分析,採取相應的控製措施,提高放療重複襬位精度.
목적 재입체정향방사치료중,대불동부위종류환자파위오차진행측량여분석,탐토기영향인소급공제방법.방법 선취54례종류환자,기중17례뇌전이환자,27례전이성혹원발성폐암환자,10례전이성간암환자,행입체정향방사치료,치료전행CBCT소묘,병결합소묘결과학정파구,대파구중심재X、Y、Z축상적파위오차진행통계분석.결과 두부파위오차교소,기오차평균치<1mm,이흉복부환자적파위오차칙상대교대,평균치<3 mm,단두부화흉복부적파위오차균공제재정학방사치료적요구범위내;대X축、Y축이급Z축삼개방향적오차범위화평균치래간,흉복부방사치료적환자비두부방사치료적환자파위중복성준학솔저.결론 흉복부파위겁역수환자적호흡、체질량변화이급방광충영도등인소적영향이발생변화,통과대영향인소진행종합분석,채취상응적공제조시,제고방료중복파위정도.
Objective To measure and analyze positioning error of stereotactic radiation therapy for patients with cancer in different position,and to investigate the influencing factors and control methods.Methods Selected 54 patients and conducted stereotactic radiotherapy treatment,including 17 cases of brain metastases,27 cases of metastatic or primary lung cancer and 10 patients with metastatic liver cancer.CBCT scan was performed,and the target was determined according to the scan results.The positio ning error of the target center in X,Y,Z axis were statistical analyzed.Results Head positioning error is small,and the error is less than the average of 1 mm,and the patient' s chest and abdomen setup errors are relatively large,in the average of within 3 mm,and the head and chest and abdomen bit errors are controlled precision radiotherapy within the required range.the X-axis,Y-axis,Z-axis and the average of the three directions,and the error range of the Z axis,the positioning repeatability in patients with thoracic and abdominal radiation therapy were in lower accuracy than patients with head radiation therapy.Conclusions Thoracic and abdominal position can be easily affected by patient's breathing,weight changes,bladderand filling and other factors.Through a comprehensive analysis of the factors,appropriate measures are suggested to be taken to improve the accuracy of radiotherapy repeat positioning.
