目的:分析腹腔镜与开腹胃癌手术对患者术后免疫功能及细胞因子含量的影响差异。方法:选择114例接受手术治疗的胃癌患者,将其分为接受腹腔镜手术治疗的观察组和接受开腹手术治疗的对照组,比较两组患者的手术一般指标、治疗前后免疫功能T淋巴细胞亚群(CD3+、CD4+及CD4+/CD8+)、细胞因子(TNF-α、IFN-γ)和白细胞介素(IL-6及IL-10)水平差异。结果:①观察组患者接受腹腔镜手术的失血量少于对照组,术后排气时间、下床时间及总住院时间均明显短于对照组,差异有统计学意义(t=5.392,t=4.521,t=5.395,t=4.937;P<0.05);②观察组患者接受腹腔镜手术治疗后的CD3+、CD4+及CD4+/CD8+水平均明显高于对照组,差异有统计学意义(t=4.281,t=5.022,t=4.672;P<0.05);③观察组患者接受腹腔镜手术治疗后的IL-6和IL-10水平低于对照组,而TNF-α及IFN-γ水平明显高于对照组,差异有统计学意义(t=4.294,t=5.138,t=8.362,t=7.173;P<0.05)。结论:腹腔镜手术有助于减少胃癌患者的手术创伤,促进术后康复,提升术后免疫功能并优化细胞因子水平。

목적:분석복강경여개복위암수술대환자술후면역공능급세포인자함량적영향차이。방법:선택114례접수수술치료적위암환자,장기분위접수복강경수술치료적관찰조화접수개복수술치료적대조조,비교량조환자적수술일반지표、치료전후면역공능T림파세포아군(CD3+、CD4+급CD4+/CD8+)、세포인자(TNF-α、IFN-γ)화백세포개소(IL-6급IL-10)수평차이。결과:①관찰조환자접수복강경수술적실혈량소우대조조,술후배기시간、하상시간급총주원시간균명현단우대조조,차이유통계학의의(t=5.392,t=4.521,t=5.395,t=4.937;P<0.05);②관찰조환자접수복강경수술치료후적CD3+、CD4+급CD4+/CD8+수평균명현고우대조조,차이유통계학의의(t=4.281,t=5.022,t=4.672;P<0.05);③관찰조환자접수복강경수술치료후적IL-6화IL-10수평저우대조조,이TNF-α급IFN-γ수평명현고우대조조,차이유통계학의의(t=4.294,t=5.138,t=8.362,t=7.173;P<0.05)。결론:복강경수술유조우감소위암환자적수술창상,촉진술후강복,제승술후면역공능병우화세포인자수평。
Objective: To analysis Effect between laparoscopic and open surgery on gastric cancer patients’ immune function and Cytokine levels. Methods:Chosen gastric cancer patients in our hospital patients as research object,divided into observation group with laparoscopic surgery and control group with open operation,compared general index, immune function(CD3+, CD4+, CD4+/CD8+) and cytokines(TNF-α, IFN-γ, IL-6, IL-10) level. Results:1)observation group patients’ blood loss was less than control group,postoperative exhaust time,bed time and total length of hospital stay were significantly shorter than control group (t=5.392, t=4.521, t=5.395, t=4.937; P<0.05); 2)observation group patients’ CD3+, CD4+, CD4+/CD8+ levels were significantly higher than control group patients(t=4.281, t=5.022, t=4.672; P<0.05); 3)observation group patients’ IL-6, IL-10 level were lower than control group patients,and TNF-α and level of IFN-γ were significantly higher than control group patients(t=4.294, t=5.138, t=8.362, t=7.173;P<0.05). Conclusion:laparoscopic surgery can help reduce the surgical trauma patients with gastric cancer, promote postoperative recovery, enhance the level of postoperative immune function and optimization of cytokines.
