目的 观察连续性肾脏替代治疗(CBP)对脓毒血症伴有急性呼吸窘迫综合征患者的影响.方法 以2009年2月至2014年2月在胶州市人民医院ICU住院治疗的脓毒血症伴急性呼吸窘迫综合征的50例患者为观察对象,随机分为治疗组、对照组两组,接受CBP患者为治疗组,除此之外,两组接受相同的治疗与护理.监测患者血管外肺水指数、血流动力学指标及相关血生化指标.结果 两组患者在进入ICU时各项指标均高于正常,差异未见统计学意义.经过CBP治疗后,治疗组血管外肺水指数、氧代谢、血生化指标好于对照组,死亡率低于对照组,差异有统计学意义(P<0.05).结论 CBP可以降低患者的肺水肿程度,改善氧代谢,提高治疗有效性.

목적 관찰련속성신장체대치료(CBP)대농독혈증반유급성호흡군박종합정환자적영향.방법 이2009년2월지2014년2월재효주시인민의원ICU주원치료적농독혈증반급성호흡군박종합정적50례환자위관찰대상,수궤분위치료조、대조조량조,접수CBP환자위치료조,제차지외,량조접수상동적치료여호리.감측환자혈관외폐수지수、혈류동역학지표급상관혈생화지표.결과 량조환자재진입ICU시각항지표균고우정상,차이미견통계학의의.경과CBP치료후,치료조혈관외폐수지수、양대사、혈생화지표호우대조조,사망솔저우대조조,차이유통계학의의(P<0.05).결론 CBP가이강저환자적폐수종정도,개선양대사,제고치료유효성.
Objective To observe the effect of continuous renal replacement therapy on sepsis patients with acute respiratory distress syndrome.Methods From February 2009 to February 2014 at the Jiaozhou City People's hospital,the 50 patients with respiratory distress syndrome sepsis in acute ICU were chosen as the observation objects and were randomly divided into treatment group and control group,patients receiving continuous renal replacement therapy were selected as the treatment group;in addition,the two groups received the same treatment and nursing.Extravascular lung water index,index of hemodynamics and blood biochemical indexes were monitored.Results The two groups in the ICU indexes were higher than normal,without significant difference between them.After CBP treatment,the extravascular lung water index,oxygen metabolism and blood biochemical index improved compared to the control group,the mortality rate was lower in the treatment group than that in the control group,there was significant difference (P < 0.05).Conclusions CBP can reduce the degree of pulmonary edema,improve the oxygen metabolism,and the efficiency of treatment.
