目的:分析与探讨老年葡萄球菌感染肺炎应用去甲万古霉素后血药浓度监测指导临床合理用药的价值。方法选取我院收治的葡萄球菌肺炎80例,根据年龄将其分为2组,老年组50例,中青年组30例。给予去甲万古霉素进行静脉滴注,治疗3 d后对其血药浓度进行监测,并根据监测结果调整给药方式与给药剂量以指导临床合理用药。比较调整前后血肌酐水平与血尿素氮水平。结果老年组应用去甲万古霉素后的血药浓度变化与中青年组相比差异有统计学意义(P<0.05)。调整给药方式与给药剂量后,血肌酐与血尿氮素水平均趋于稳定。结论老年患者使用去甲万古霉素进行治疗过程中监测血药浓度能够有效指导合理用药,降低药物损害。

목적:분석여탐토노년포도구균감염폐염응용거갑만고매소후혈약농도감측지도림상합리용약적개치。방법선취아원수치적포도구균폐염80례,근거년령장기분위2조,노년조50례,중청년조30례。급여거갑만고매소진행정맥적주,치료3 d후대기혈약농도진행감측,병근거감측결과조정급약방식여급약제량이지도림상합리용약。비교조정전후혈기항수평여혈뇨소담수평。결과노년조응용거갑만고매소후적혈약농도변화여중청년조상비차이유통계학의의(P<0.05)。조정급약방식여급약제량후,혈기항여혈뇨담소수평균추우은정。결론노년환자사용거갑만고매소진행치료과정중감측혈약농도능구유효지도합리용약,강저약물손해。
Objective To analyze and investigate elderly patients norepinephrine guide rational drug use behavior after the value of therapeutic drug monitoring of vancomycin. Methods A total of 80 cases of patients with pneumonia in our hospital from August 2013 to August 2014 were divided into two groups according to age : the older group had 50 cases, the young group had 30 cases. Patients were given intravenous vancomycin treatment monitoring its blood concentration after 3 d, and mode of administration and dose adjustment based on monitoring results to guide rational drug use behavior. Serum creatinine and blood urea nitrogen levels in patients were compared before and after the adjustment. Results Changes in plasma concentration after norvancomycin had a difference between the elderly patients and young patients(P<0.05). The serum creatinine and urine nitrogen levels were stabilizing after the mode of administration and dosage adjustment.Conclusion In the course of elderly patients to vancomycin for treatment, the use of therapeutic drug monitoring can effectively guide clinical rational use of drugs to reduce the damage caused by the drug to the patient.
