目的:观察上颌窦内提升术后种植体植入存留情况以及种植体周围骨量变化情况,为临床应用该技术提供可靠依据。方法选择我科行种植修复患者110例,根据是否行上颌窦提升分为2组。试验组采用上颌窦内提升方式进行上颌窦提升并植入Bio-Oss骨粉,同期进行种植体植入。对照组采用常规操作植入种植体。所有入组患者术后不同时间点进行全口曲面断层检查,牙周探诊深度检查(probing pocket depth,PPD)、探诊出血指数(bleeding on probing,BOP)、菌斑指数(plaque index,PI)检查,种植体存留情况,种植体根方至窦底实际骨高度(actual bone hight,ABH)。应用SPSS 15.0统计学软件包进行数据统计学分析。结果试验组55例,植入76颗种植体;对照组55例,植入70颗种植体。2组修复后复查,种植体脱落率、PPD深度、BOP指数、PI指数差异均无统计学意义(P<0.05)。试验组ABH术后即刻与术后36个月相比差异有统计学意义(P<0.05)。结论上颌窦内提升技术为种植体植入骨量不足的患者提供了良好的治疗方案,且能保证种植体较高的存留率,以及良好的植体周围健康。

목적:관찰상합두내제승술후충식체식입존류정황이급충식체주위골량변화정황,위림상응용해기술제공가고의거。방법선택아과행충식수복환자110례,근거시부행상합두제승분위2조。시험조채용상합두내제승방식진행상합두제승병식입Bio-Oss골분,동기진행충식체식입。대조조채용상규조작식입충식체。소유입조환자술후불동시간점진행전구곡면단층검사,아주탐진심도검사(probing pocket depth,PPD)、탐진출혈지수(bleeding on probing,BOP)、균반지수(plaque index,PI)검사,충식체존류정황,충식체근방지두저실제골고도(actual bone hight,ABH)。응용SPSS 15.0통계학연건포진행수거통계학분석。결과시험조55례,식입76과충식체;대조조55례,식입70과충식체。2조수복후복사,충식체탈락솔、PPD심도、BOP지수、PI지수차이균무통계학의의(P<0.05)。시험조ABH술후즉각여술후36개월상비차이유통계학의의(P<0.05)。결론상합두내제승기술위충식체식입골량불족적환자제공료량호적치료방안,차능보증충식체교고적존류솔,이급량호적식체주위건강。
Objective To evaluate the survival rate of dental implants and changes of peri-implant status after maxillary sinus lifting. Methods Patients were separated into experimental group and control group according to the fact whether maxillary sinus floor elevation was placed or not. In experimental group, patients were placed implants simultaneously after maxillary sinus floor elevation and bone graft. In control group, implants were placed directly without maxillary sinus lifting and bone graft. X ray was taken and probing pocket depth(PPD), bleeding on probing(BOP) were measured and plaque Index(PI) of all implants had been placed on the day of operation, 6, 12 and 36 months later. The survival rate of implants was counted. The actual bone hight(ABH) from implant root to the bottom of the maxillary sinus was calculated. Statistical analysis with SPSS 15.0 package was carried out. Results In experimental group, 55 patients were placed 76 implants. In control group, 55 patients were placed 70 implants. No significant differences were observed in the survival rates, PPD, BOP, PI in two groups(P<0.05). ABH changes showed obvious bone resorption(P<0.05). Conclusion This study showed maxillary sinus floor elevation was a good technique for patients with maxillary bone deficiency. High survival rate and healthy peri-implant status could be ensured.
