目的：探讨 ApacheⅡ分阶联合核心因素思维判断重症患者入住时机的效果。方法：在回顾性分析及专家咨询的基础上确定核心因素思维变量及分值，选择1091例患者进行随机对照研究，观察应用效果。结果：实验组与对照组在评分分组 A、B、C 组中病死率比较差异有统计学意义（P <0.05）。结论：ApacheⅡ分阶联合核心因素思维判断重症患者入住 ICU 时机可以帮助医护人员快速区分危重患者，指导护士建立临床护理思维，提升实践能力，加强普通病房、监护病房、ICU 之间良好配合及对接。

Objective:To study the effect of acute physiology and chronic health evaluation Ⅱ(Apache Ⅱ)scoring combined with key factor thinking in the judgment of the time of admission to ICU in critically ill patients. Methods:The randomized controlled trial was made on 1 091 patients,using the variables and scores of key factor thinking determined based on the retrospective analysis and expert's consul-tation,and then the application effect was observed. Results:There was statistically significant difference in the comparison of the case fa-tality rate between the experimental group and the control group(P < 0. 05). Conclusion:Application of Apache Ⅱ scoring combined with key factor thinking in the judgment of the time of admission to ICU in critically ill patients can help medical personnel quickly distinguish critically ill patients and guide nurses establish clinical nursing thinking,enhance practical ability and strengthen coordination and connec-tion between wards and ICU.
