目的 探讨CT能谱多参数成像对肝癌的诊断价值.方法 选取2012年4月至2014年5月在我院行肝脏能谱CT扫描的33例患者,对肝癌患者的能谱定量参数进行分析.结果 肝癌患者的动脉期与门静脉期的40、50、120 keV下的CT值差异有统计学意义(P<0.05),该两期的CT值随着keV增高而降低;肝癌动脉期能谱CT值衰减曲线斜率为(1.76±0.43),门静脉期为(2.79±0.55),动脉期和门静脉期的能谱CT衰减曲线斜率差异有统计学意义(P<0.05);动脉期肝癌在50 keV下CNR最佳,门静脉期肝癌在70 keV下具有最佳的CNR;肝癌动脉期NIC低于门静脉期,动脉期ICRLN高于门静脉期,而两期脂肪/钙浓度和水(碘)浓度差异无统计学意义(P>0.05).结论 CT能谱多参数成像在诊断肝癌中有较高的诊断价值.

Objective To explore the value of spectral imaging of CT multi-parameters for diagnosing liver cancer.Methods 33 patients with liver cancer undertaking spectral imaging of CT at our hospital from April,2012 to May,2014 were selected.Their energy spectrum parameters were analyzed.Results There were statistical differences (P<0.05) in CT value between the patients in arterial and portal venous phases at 40,50,and 120 keV.The CT value decreased with the increase of keV in these two phases.The slope of attenuation curve of CT value in liver cancer arterial phase was (1.76±0.43) and portal venous phase (2.79±0.55),with a statistical differences (P<0.05).The CNR in arterial phase of liver cancer was best at 50 keV and in portal venous phase at 70 keV.NIC was lower and ICRLN was higher in arterial phase than in venous phase of liver cancer.There was no statistical differences(P > 0.05) in fat/calcium concentration and water (iodine) concentration between these two phases.Conclusion Spectral imaging of CT multi-parameters has high value in the diagnosis of liver cancer.
