【目的】探讨妊娠糖尿病(GDM )患者妊娠期血脂水平及其与孕期体质量变化的关系。【方法】选择159例妊娠24~28周孕妇,根据口服葡萄糖糖耐量试验(OGTT)结果,将其分为GDM 组(n=70)及健康孕妇组(n=89);记录孕前、孕期体质量,测量身高,计算孕前及孕期体质量指数(BMI)、孕期增重;测定血浆总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL‐C)、低密度脂蛋白胆固醇(LDL‐C)、空腹胰岛素(FINS)、糖化血红蛋白(HbA1c)等水平;比较两组间血脂水平、体质量变化的差异及相关性。【结果】GDM 组 TC、LDL‐c显著低于健康孕妇组(均 P <0.05),HDL‐C显著高于健康孕妇组( P <0.05),健康孕妇组TG水平高于GDM 组,但无显著性差异( P>0.05);但健康孕妇中,TC与体质量增加( r =0.249,P=0.017)、孕期BMI( r=0.224,P=0.033)及BMI增加( r =0.259,P=0.013)相关;GDM 组中,TC与BMI( P =0.241,P =0.041)相关。【结论】GDM患者存在血脂代谢紊乱,但孕期营养摄入及体质量增加过多可能对血脂的影响更大。

【목적】탐토임신당뇨병(GDM )환자임신기혈지수평급기여잉기체질량변화적관계。【방법】선택159례임신24~28주잉부,근거구복포도당당내량시험(OGTT)결과,장기분위GDM 조(n=70)급건강잉부조(n=89);기록잉전、잉기체질량,측량신고,계산잉전급잉기체질량지수(BMI)、잉기증중;측정혈장총담고순(TC)、삼선감유(TG)、고밀도지단백담고순(HDL‐C)、저밀도지단백담고순(LDL‐C)、공복이도소(FINS)、당화혈홍단백(HbA1c)등수평;비교량조간혈지수평、체질량변화적차이급상관성。【결과】GDM 조 TC、LDL‐c현저저우건강잉부조(균 P <0.05),HDL‐C현저고우건강잉부조( P <0.05),건강잉부조TG수평고우GDM 조,단무현저성차이( P>0.05);단건강잉부중,TC여체질량증가( r =0.249,P=0.017)、잉기BMI( r=0.224,P=0.033)급BMI증가( r =0.259,P=0.013)상관;GDM 조중,TC여BMI( P =0.241,P =0.041)상관。【결론】GDM환자존재혈지대사문란,단잉기영양섭입급체질량증가과다가능대혈지적영향경대。
[Objective]To explore the relationship between serum lipids and weight changes in pregnant women with gestational diabetes mellitus (GDM ) .[Methods]Pregnant women at 24~28 weeks of gestation were divided into GDM ( n=70) and healthy pregnant women (n=89) according to the results of oral glucose tolerance test (OGTT) .Weight at pre‐pregnancy and at 24~28 weeks of gestation and height were recorded .Body mass index (BMI) and weight gain were calculated and the levels of total cholesterol (TC) ,triglyceride (TG) ,high‐density lipoprotein cholesterol (HDL‐c) ,low density lipoprotein cholesterol (LDL‐c) ,fasting insulin (FINS) ,and hemoglobin A1c (HbA1c) determined .The differ‐ences and correlations were detected between lipid and weight changes .[Results]The levels of total cholesterol (TC) and low‐density lipoprotein‐cholesterol (LDL‐c) were lower and the level of high‐density lipoprotein‐cholesterol (HDL‐c) was higher in GDM ( P<0 .05) .And no significant inter‐group difference existed in triglyceride (TG) .There was a declining trend of TG in GDM .In addition ,TG was positively correlated with weight gain ( r=0 .249 ,P=0 .249) ,BMI at 24~28 weeks of gestation ( r= 0 .224 ,P=0 .033) and BMI gain ( r=0 .259 ,P=0 .013) in healthy pregnant women .TG was positively correlated with BMI in GDM at 24~28 weeks of gestation ( r=0 .241 ,P=0 .241) .[Conclusion] There is a disorder of lipid metabolism in GDM .And a larger nutrient intake and weight gain have greater effects .
