【目的】探讨内镜下尼龙绳结扎术在预防胃肠道宽基及粗蒂息肉电凝切术并发症的作用及意义。【方法】74例患者随机分为两组,观察组38例采用先内镜下尼龙绳结扎胃肠道宽基底及粗蒂息肉,后行息肉电凝切术;对照组36例单纯内镜下息肉电凝切术。观察并比较两组术中出血、穿孔情况及术后14 d内消化道出血、息肉残留等并发症情况。【结果】观察组14 d内出现并发症2例(出血1例,息肉残留1例);对照组出现并发症10例(出血6例,穿孔2例,息肉残留2例),两组比较有显著性差异( P <0.05)。【结论】内镜下尼龙绳结扎术在可有效预防胃肠道宽基及粗蒂息肉电切术后并发症的发生,具有疗效肯定、安全等优点。

【목적】탐토내경하니룡승결찰술재예방위장도관기급조체식육전응절술병발증적작용급의의。【방법】74례환자수궤분위량조,관찰조38례채용선내경하니룡승결찰위장도관기저급조체식육,후행식육전응절술;대조조36례단순내경하식육전응절술。관찰병비교량조술중출혈、천공정황급술후14 d내소화도출혈、식육잔류등병발증정황。【결과】관찰조14 d내출현병발증2례(출혈1례,식육잔류1례);대조조출현병발증10례(출혈6례,천공2례,식육잔류2례),량조비교유현저성차이( P <0.05)。【결론】내경하니룡승결찰술재가유효예방위장도관기급조체식육전절술후병발증적발생,구유료효긍정、안전등우점。
[Objective] To evaluate the preventive effectiveness of endoscopic nylon endoloop for complications during electric coagulation electrocision of broad base and thick pedicle polyp in gastrointestinal tract .[Methods]A total of 74 patients were randomly divided into two groups .For treatment group ( n =38) ,endoscopic nylon endoloop was applied before electric coagulation electrocision of broad base and thick pedicle polyp in gastrointesti‐nal tract .For control group ( n=36) ,only endoscopic electric coagulation electrocision was performed .The oc‐currences of hemorrhages or perforation during operations and hemorrhages of digestive tract or residual polyp up until Week 2 were observed and recorded .[Results] At Week 2 after treatment ,complications occurred in treat‐ment group (hemorrhage ,n=1;residual polyp ,n=1) and control group (hemorrhage ,n=6 ;perforation ,n =2;residual polyp ,n =2) .Statistically significant differences existed between two groups ( P <0 .05) .[Conclu‐sion]:Endoscopic nylon endoloop can effectively and safely prevent the occurrences of complications during electric coagulation electrocision of broad‐base‐and‐thick‐pedicle polyps in gastrointestinal tract .
