目的：分析宫颈扩张球囊联合缩宫素及间苯三酚在足月妊娠引产中的应用效果。方法选取本院妇产科2012年1月~2014年11月接收的136例符合引产指征的的足月妊娠初产妇，根据其意愿将其分为对照组(n=74)和观察组(n=62)。对照组患者于宫颈评分后给予0.5%缩宫素，潜伏期给予间苯三酚；观察组在对照组基础上放置Cook双球囊。比较两组产妇干预前后的宫颈 Bishop评分；干预12 h后，比较两组产妇的促宫颈成熟效果；比较两组产妇的引产效果、分娩方式及妊娠结局；比较两组产妇的产程情况。结果干预前，两组的宫颈 Bishop评分比较差异无统计学意义(P>0.05)，干预后，观察组显著高于对照组(P<0.05)。观察组的促宫颈成熟效果显著优于对照组(P<0.05)。观察组12 h内临产、阴道分娩的比例显著高于对照组(P<0.05)，产后尿潴留、产后出血、胎儿窘迫、新生儿窒息等妊娠结局发生率显著低于对照组(P<0.05)。除第二产程外，观察组产妇的产程均显著短于对照组(P<0.05)。结论宫颈扩张球囊联合缩宫素及间苯三酚能够提高宫颈 Bishop评分，提高阴道分娩成功率，降低不良妊娠结局发生率。
목적：분석궁경확장구낭연합축궁소급간분삼분재족월임신인산중적응용효과。방법선취본원부산과2012년1월~2014년11월접수적136례부합인산지정적적족월임신초산부，근거기의원장기분위대조조(n=74)화관찰조(n=62)。대조조환자우궁경평분후급여0.5%축궁소，잠복기급여간분삼분；관찰조재대조조기출상방치Cook쌍구낭。비교량조산부간예전후적궁경 Bishop평분；간예12 h후，비교량조산부적촉궁경성숙효과；비교량조산부적인산효과、분면방식급임신결국；비교량조산부적산정정황。결과간예전，량조적궁경 Bishop평분비교차이무통계학의의(P>0.05)，간예후，관찰조현저고우대조조(P<0.05)。관찰조적촉궁경성숙효과현저우우대조조(P<0.05)。관찰조12 h내임산、음도분면적비례현저고우대조조(P<0.05)，산후뇨저류、산후출혈、태인군박、신생인질식등임신결국발생솔현저저우대조조(P<0.05)。제제이산정외，관찰조산부적산정균현저단우대조조(P<0.05)。결론궁경확장구낭연합축궁소급간분삼분능구제고궁경 Bishop평분，제고음도분면성공솔，강저불량임신결국발생솔。
Objective To analyze the application effect of cervical dilatation ballooncombined with oxytocin and phloroglucinol in induced labour in full-term pregnancy. Methods 136 full-term pregnancy primipara met the criteria of induced labor treated in department of gynaecology and obstetrics from January 2012 to November 2014 were select-ed and they were divided into control group (n=74) and observation group (n=62) based on their will.In control group, 0.5% oxytocin was provided after cervical score and phloroglucinol was used in latent period.In observation group,on the basis of the control group,Cook double balloon was placed.Cervical Bishop score before and after intervention was compared.Cervical ripening effect in puerperae in two groups was compared after 12 hours intervention.The induction effect,delivery mode,and pregnancy outcome in these puerperae between two groups were compared as well as stage of labor. Results Before intervention,there was no statistical difference of cervical Bishop score between two groups (P>0.05).After intervention,the score in observation group was much higher than that in control group (P<0.05).Cervical ripening effect in observation group was much better than that in control group (P<0.05).The proportion of patients be-ing going to give birth with wtthin 12 hours and patients preferred for vaginal delivery was greatly higher than those in control group (P<0.05).Incidence rate of pregnancy outcome like postpartum urinary retention,postpartum hemorrhage, fetal distress,neonatal asphyxia,etc.were significantly lower than those in control group (P<0.05).Except the second stage of labor, stages of labor in observation group were shorter than those in the control group (P<0.05). Conclusion Cervi-cal dilatation balloon combined with oxytocin and phloroglucinol can increase cervical Bishop score,improve the suc-cess rate of vaginal delivery,and reduce the incidence rate of adverse pregnancy outcome.
