目的:观察右美托咪啶对肺癌根治术患者肺功能的影响。方法:选取60例择期行肺癌根治术患者,随机分为实验组和对照组,实验组采用右美托咪啶的方法进行预处理,对照组采用安慰剂生理盐水进行预处理。两组患者分别于单肺通气后30 min(T1)、单肺通气后90 min(T2)、术后1 d(T3)三个时点采集静脉血检测白细胞介素-1β(IL -1β)、白细胞介素-10(IL -10)浓度,同时采集动脉血行血气分析。结果:与对照组比较,实验组 T2,T3时点 IL -1β显著降低(P <0.05),IL -10显著升高(P <0.05)。实验组患者术后动脉血氧分压(PaO2)显著高于对照组,而二氧化碳分压(PaCO2)显著低于对照组(P <0.05)。结论:右美托咪啶预处理可抑制促炎性细胞因子的表达,促进抗炎性细胞因子的释放,且能改善患者术后肺交换功能,对患者肺功能有一定保护作用。

목적:관찰우미탁미정대폐암근치술환자폐공능적영향。방법:선취60례택기행폐암근치술환자,수궤분위실험조화대조조,실험조채용우미탁미정적방법진행예처리,대조조채용안위제생리염수진행예처리。량조환자분별우단폐통기후30 min(T1)、단폐통기후90 min(T2)、술후1 d(T3)삼개시점채집정맥혈검측백세포개소-1β(IL -1β)、백세포개소-10(IL -10)농도,동시채집동맥혈행혈기분석。결과:여대조조비교,실험조 T2,T3시점 IL -1β현저강저(P <0.05),IL -10현저승고(P <0.05)。실험조환자술후동맥혈양분압(PaO2)현저고우대조조,이이양화탄분압(PaCO2)현저저우대조조(P <0.05)。결론:우미탁미정예처리가억제촉염성세포인자적표체,촉진항염성세포인자적석방,차능개선환자술후폐교환공능,대환자폐공능유일정보호작용。
Objective:To observe the effect of dexmedetomidine on pulmonary function in patients undergoing radical sur-gery for lung cancer. Methods:Sixty patients undergoing radical resection of lung cancer were randomly divided into experimen-tal group and control group,the experimental group method using dexmedetomidine pretreatment,the control group using saline placebo pretreatment. The interleukin - 1β(IL - 1 β),interleukin - 10(IL - 10)of two group was detected in 30 min after one lung ventilation(T1),90 min after one lung ventilation(T2),1 postoperative days(T3)three point,and arterial blood gas analysis . Results :Compared with the control group ,T 2 ,T 3 IL - 1 β in the experimental group decreased significantly( P <0. 05),IL - 10 increased significantly(P < 0. 05). The alveolar oxygen partial pressure(PaO2 )of experimental group was sig-nificantly higher than that in control group after surgery,while alveolar partial pressure of carbon dioxide(PaCO2 )was signifi-cantly lower than the control group(P < 0. 05). Conclusion:Dexmedetomidine pretreatment can inhibit the expression of proin-flammatory cytokines,promote anti - inflammatory cytokine release,and can improve the postoperative lung function exchange, has certain protective effect on lung function in patients.
