目的:利用彩色超声多普勒成像(CDI)技术对糖尿病视网膜病变(DR)患者球后血管血流动力学变化进行相关研究。方法选取确诊为糖尿病(DM)的患者纳入DM组,分为无视网膜病变(NDR)组、非增殖性视网膜病变(NPDR)组、增殖性视网膜病变(PDR)组。设立健康对照(HC)组,应用CDI检测研究对象颈动脉(CA)、眼动脉(OA)、视网膜中央动脉(CRA)及视网膜中央静脉(CRV)血流参数并进行比较分析。结果与HC组比较,NDR期CA、OA血流及颈动脉中内膜厚度(IMT)变化不明显,但CRA、CRV出现收缩期峰值流速(PSV)和舒张末期血流速度(EDV)增加(P<0.05);NPDR及PDR期CA、OA及CRA的PSV、EDV呈下降趋势(P<0.05),阻力指数(RI)、搏动指数(PI)及IMT增高(P<0.05)。结论 CDI技术对DR早期诊断有一定参考价值,虽在准确判断视网膜血液循环方面还存在局限性,但可为判断预后及分析病变提供重要信息。

목적:이용채색초성다보륵성상(CDI)기술대당뇨병시망막병변(DR)환자구후혈관혈류동역학변화진행상관연구。방법선취학진위당뇨병(DM)적환자납입DM조,분위무시망막병변(NDR)조、비증식성시망막병변(NPDR)조、증식성시망막병변(PDR)조。설립건강대조(HC)조,응용CDI검측연구대상경동맥(CA)、안동맥(OA)、시망막중앙동맥(CRA)급시망막중앙정맥(CRV)혈류삼수병진행비교분석。결과여HC조비교,NDR기CA、OA혈류급경동맥중내막후도(IMT)변화불명현,단CRA、CRV출현수축기봉치류속(PSV)화서장말기혈류속도(EDV)증가(P<0.05);NPDR급PDR기CA、OA급CRA적PSV、EDV정하강추세(P<0.05),조력지수(RI)、박동지수(PI)급IMT증고(P<0.05)。결론 CDI기술대DR조기진단유일정삼고개치,수재준학판단시망막혈액순배방면환존재국한성,단가위판단예후급분석병변제공중요신식。
Objective To study the changes of retrobulbar vascular hemodynamics in patients with diabetic retinopathy (DR) by using color Doppler imaging technology (CDI). Methods The diagnosed patients with diabetic mellitus (DM) were divided into non-diabetic retinopathy (NDR) group, non-proliferative diabetic retinopathy (NPDR) group and proliferative diabetic retinopathy (PDR) group. The healthy patients were served as control (HC) group. The states of carotid artery (CA), ophthalmic artery (OA), central retinal artery (CRA) and central retinal vein (CRV) blood lfow parameters were examined with CDI and the results were analyzed. Results Compared with HC group, the blood lfow of carotid artery(CA), ophthalmic artery(OA) and the intima-media thickness (IMT) changes were not obvious in the patients with non-diabetic retinopathy(NDR), but the appearance of CRA peak systolic velocity (PSV) and end diastolic velocity (EDV)increased (P<0.05);phase NDR, CRV increased velocity (P<0.05). In NPDR group and PDR group, PSV, EDV in CA, OA and CRA decreased (P<0.05), resistance index (RI) and pulsatility index (PI) increased (P<0.05). Conclusion There is a reference value for the early diagnosis of DR using color Doppler imaging, but it has limitations in the accurate judgment of retinal blood circulation, however, it can be applied to provide some important information for prognosis and analysis of the degree of lesion.
