目的：比较手术切除和射频消融治疗根治性切除术后复发性小肝癌（≤3cm）的临床疗效。方法52例小肝癌患者，手术切除25例，射频消融27例，分析临床资料及随访结果。结果全组中位随访时间27（8~43）个月。手术组与射频组比较1、2、3年无瘤生存率为76.0%、48.0%、32.0%vs 77.8%、51.9%、37.1%，平均无瘤生存时间为21.4个月vs 23.5个月，总1、2、3年累积生存率为92.0%、80.0%、68.0%vs 92.6%、74.1%、63.0%，平均生存时间为35.8个月vs 32.7个月。无瘤生存率与累积生存率差别均无统计学意义。结论射频消融治疗对于直径≤3cm的切除术后复发性小肝癌疗效确切，近远期疗效与手术切除相近，可以考虑作为其首选的治疗手段之一。

Objective To compare clinical results on surgical resection(SR) and radiofrequency ablation(RFA) for the treatment of the patients with recurrence small primary liver carcinoma with the diameter smaller than 3 cm after curative resection. Methods The clinical results on 52 patients were analyzed. Of 52 patients,25 underwent surgical resection and 27 did radiofrequency ablation. Results With media follow-up time 27(8~43) months, the 1,2,3-year disease free survival rates in SR and RFA group were 76.0%,48.0%and 32.0%vs 77.8%,51.9%and 37.1%, the mean disease free survival time were 21.4 vs 23.5months. The 1,2,3-year cumulative survival rates in SR and RFA group were 92.0%,80.0%and 68.0%vs 92.6%,74.1%and 63.0%, the mean cumulative survival time were 35.8 vs 32.7 months. The 1,2,3-year disease free survival rates and cumulative survival rates in SR and RFA group were statistically not significant. Conclusion Radiofreqency ablation is a safe and satisfactory treatment for the treatment of the patients with recurrence small liver carcinoma with the diameter smaller than 3 cm after curative resection with short and long-time results that similar to surgical resection, which can be selected as one of the first-line treatments.
