目的:探讨变异血红蛋白对两种基于HPLC方法测定糖化血红蛋白结果的干扰。方法选取我院诊断的6种不同类型血红蛋白变异者血液及婴儿脐带血共60份作为实验组,同时选取我院体检健康者血液60份作为对照组,分别使用AC-HPLC和IE-HPLC系统对两组HbA1C进行检测,探讨7种不同变异血红蛋白对糖化血红蛋白两种检测方法的干扰并分析其原因。结果在AC-HPLC检测中,各变异血红蛋白,与对照组相比,均无明显差异,其HbA1C均与受测人员血糖浓度呈正相关。而在IE-HPLC检测中,HbD、E、G、J、Q、S均明显高于AC-HPLC和对照组中HbA1C水平,差异有统计学意义(P<0.05),但HbF变异蛋白检测中未检出HbA1C。结论 IE-HPLC系统检测糖化血红蛋白时容易受变异血红蛋白干扰,而AC-HPLC系统则不受其影响,值得推广。

목적:탐토변이혈홍단백대량충기우HPLC방법측정당화혈홍단백결과적간우。방법선취아원진단적6충불동류형혈홍단백변이자혈액급영인제대혈공60빈작위실험조,동시선취아원체검건강자혈액60빈작위대조조,분별사용AC-HPLC화IE-HPLC계통대량조HbA1C진행검측,탐토7충불동변이혈홍단백대당화혈홍단백량충검측방법적간우병분석기원인。결과재AC-HPLC검측중,각변이혈홍단백,여대조조상비,균무명현차이,기HbA1C균여수측인원혈당농도정정상관。이재IE-HPLC검측중,HbD、E、G、J、Q、S균명현고우AC-HPLC화대조조중HbA1C수평,차이유통계학의의(P<0.05),단HbF변이단백검측중미검출HbA1C。결론 IE-HPLC계통검측당화혈홍단백시용역수변이혈홍단백간우,이AC-HPLC계통칙불수기영향,치득추엄。
Objective To study the interference of variability hemoglobin in the detection results of glycosylated hemoglobin of the two HPLC methods. Methods 60 patients with 6 kinds of different types of variability hemoglobin and infant umbilical cord blood who were diagnosed in our hospital were selected as the experimental group, while the other 60 cases of healthy blood were selected as the control group. To study and analyze the interference causes of 7 kinds of variability hemoglobin in the two detection methods.Results Of the AC-HPLC detection, all kinds of variability hemoglobin between the two groups were no significant differences, HbA1C of tested personnel was positive correlation with blood glucose concentration. HbA1C levels of HbD、E、G、J、Q、S in IE-HPLC detection were higher than which in AC-HPLC, the difference had statistical significance(P<0.05), but HbA1C wasn’t detected in HbF variability hemoglobin.Conclusion Glycosylated hemoglobin of IE-HPLC detection could easily be interfered by variability hemoglobin, while AC-HPLC detection is not affected and is worthy of popularization.
