目的：探讨用锁定钢板联合钛缆固定系统治疗人工髋关节置换术后发生假体周围骨折的临床疗效。方法回顾分析本研究从2008年1月～2014年5月收治的25例Vancouver B型和C型股骨假体周围骨折患者，依照Vancouver分类系统，其中18例属于B型骨折，7例属于C型骨折，采用锁定钢板联合钛缆固定手术，随访时评价患者康复情况、骨折愈合时间及Harris评分。结果所有患者平均随访（1.0±0.3）年，其中24例在4～8个月拍片出现骨折愈合，平均为（6.5±1.2）个月。1例因骨质疏松致内固定松动致再次行人工全髋翻修手术，Harris评分为（82.4±2.5）分。结论用锁定钢板联合钛缆固定手术治疗人工髋关节置换术后Vancouver B型和C型股骨假体周围骨折疗效良好。
목적：탐토용쇄정강판연합태람고정계통치료인공관관절치환술후발생가체주위골절적림상료효。방법회고분석본연구종2008년1월～2014년5월수치적25례Vancouver B형화C형고골가체주위골절환자，의조Vancouver분류계통，기중18례속우B형골절，7례속우C형골절，채용쇄정강판연합태람고정수술，수방시평개환자강복정황、골절유합시간급Harris평분。결과소유환자평균수방（1.0±0.3）년，기중24례재4～8개월박편출현골절유합，평균위（6.5±1.2）개월。1례인골질소송치내고정송동치재차행인공전관번수수술，Harris평분위（82.4±2.5）분。결론용쇄정강판연합태람고정수술치료인공관관절치환술후Vancouver B형화C형고골가체주위골절료효량호。
Objective To study the clinical effect of locking plate combined titanium cable fixation in the treatment of periprosthetic fractures after hip replacement. Methods 25 patients with periprosthetic femoral fractures of Vancouver B-type and C-type,who were treated in the hospital from January 2008 to May 2014,were retrospectively analyzed and were classified according to Vancouver classification system with 18 cases in B-type and 7 cases in C-type.Of all the patients,to treat with locking plate combined titanium cable fixation,then to evaluate patients’rehabilitation condition,healing time,and Harris scores when follow-up. Results Of all the patients with mean follow-up period of(1.0±0.3)years,there into,24 cases had been fracture healing between 4 and 8 months when filming,with the average healing time of(6.5±1.2)months. Conclusion Locking plate combined titanium cable fixation in the treatment of periprosthetic femoral fractures of Vancouver B-type and C-type after hip replacement has well curative effect.
