目的比较微创与后传统髋关节置换术（THA）治疗髋关节晚期病变的疗效及对患者生活质量的影响，为临床治疗提供相应依据。方法收集2010年6月至2014年6月收治的140例髋关节晚期病变患者作为研究对象，随机分为微创组70例和传统组70例。微创组给予微创 THA 手术治疗，传统组给予传统 THA 手术治疗。比较两组患者手术状况、视觉模拟评级法（VAS）评分，Harris 评分以及生活质量。结果微创组切口长度显著短于传统组，术中出血量与术后引流量显著少于传统组，但手术时间显著长于传统组，差异均具有统计学意义（ P 均<0.05）。两组患者治疗后 VAS 评分较治疗前均显著降低，Harris 评分均显著升高，差异均具有统计学意义（ P 均<0.05）。治疗后，微创组 VAS评分显著低于传统组，Harris 评分显著高于传统组，差异均具有统计学意义（ P 均<0.05）。两组患者治疗后生活质量均显著提高，微创组躯体功能、机体疼痛、活力及情绪角色方面的评分均显著高于传统组，差异均具有统计学意义（ P 均<0.05）。结论微创 THA 手术切口小，创伤少，可显著减轻患者疼痛并提高其生活质量，值得临床上进一步推广应用。
목적비교미창여후전통관관절치환술（THA）치료관관절만기병변적료효급대환자생활질량적영향，위림상치료제공상응의거。방법수집2010년6월지2014년6월수치적140례관관절만기병변환자작위연구대상，수궤분위미창조70례화전통조70례。미창조급여미창 THA 수술치료，전통조급여전통 THA 수술치료。비교량조환자수술상황、시각모의평급법（VAS）평분，Harris 평분이급생활질량。결과미창조절구장도현저단우전통조，술중출혈량여술후인류량현저소우전통조，단수술시간현저장우전통조，차이균구유통계학의의（ P 균<0.05）。량조환자치료후 VAS 평분교치료전균현저강저，Harris 평분균현저승고，차이균구유통계학의의（ P 균<0.05）。치료후，미창조 VAS평분현저저우전통조，Harris 평분현저고우전통조，차이균구유통계학의의（ P 균<0.05）。량조환자치료후생활질량균현저제고，미창조구체공능、궤체동통、활력급정서각색방면적평분균현저고우전통조，차이균구유통계학의의（ P 균<0.05）。결론미창 THA 수술절구소，창상소，가현저감경환자동통병제고기생활질량，치득림상상진일보추엄응용。
Objective To compare the efficacy and effects on the life quality of minimally invasive and traditional total hip arthroplasty (THA)surgery and provide clinical basis for clinical therapy. Methods From 2010 June to 2014 June,we collected 140 cases of hip lesion in our hospital. These patients were divided into 2 groups randomly,70 cases in minimally invasive surgery group(MISG)and 70 cases in traditional surgery group(TSG). The MISG was treated with minimally invasive THA and the TSG was treated with traditional THA. The surgery status,VAS score,Harris score and life quality of the 2 groups were observed and compared. Results The MISG had significant shorter surgery time,shorter incision length,less bleeding volume and drainage flow than those of the TSG( P < 0. 05). Both 2 groups had lower VAS score,higher Harris score,and higher life quality after treatments than before treatments( P < 0. 05). The MISG had lower VAS score,higher Harris score than those of TSG( P < 0. 05). The body function,body pain,activity,and emotion role score in the life quality of the MSIG were all significantly higher than those of the TSG after treatments( P < 0. 05). Conclusion Minimally invasive THA has superiority of little trauma and can decrease the pain and improve the life quality of patients.
