目的：观察经纤维支气管镜局部化疗对中央型肺癌的治疗作用。方法：选取收治的中央型肺癌患者52例，随机分为观察组与对照组，两组患者均采用静脉CE 方案化疗，观察组在常规化疗的基础上给予经纤维支气管镜局部化疗，采用Olympus BF－1T40型纤支镜，经纤支镜活检孔置入塑胶细导管，注入卡铂400 mg喷洒至瘤体表面；对照组单纯采用静脉化疗方案治疗。观察两组化疗一疗程后的临床效果及体力状况Zubrod－ECOG－WHO评分（ ZPS）、Karnofsky 功能状态评分（ KPS）变化。结果：观察组治疗后ZPS、KPS评分均显著高于对照组（ P＜0．05）；观察组治疗后总有效率显著高于对照组（ P＜0．05），且治疗过程中毒副作用轻微。结论：经纤支镜局部化疗可显著改善中央型肺癌患者临床临床症状，缩小肿瘤体积，对中、晚期中央型肺癌治疗作用显著。
목적：관찰경섬유지기관경국부화료대중앙형폐암적치료작용。방법：선취수치적중앙형폐암환자52례，수궤분위관찰조여대조조，량조환자균채용정맥CE 방안화료，관찰조재상규화료적기출상급여경섬유지기관경국부화료，채용Olympus BF－1T40형섬지경，경섬지경활검공치입소효세도관，주입잡박400 mg분쇄지류체표면；대조조단순채용정맥화료방안치료。관찰량조화료일료정후적림상효과급체력상황Zubrod－ECOG－WHO평분（ ZPS）、Karnofsky 공능상태평분（ KPS）변화。결과：관찰조치료후ZPS、KPS평분균현저고우대조조（ P＜0．05）；관찰조치료후총유효솔현저고우대조조（ P＜0．05），차치료과정중독부작용경미。결론：경섬지경국부화료가현저개선중앙형폐암환자림상림상증상，축소종류체적，대중、만기중앙형폐암치료작용현저。
Objective:To observe the therapeutic effect of local chemotherapy by fiber bronchoscope on central type lung cancer .Methods:52 patients with central type lung cancer in our hospital were selected , randomly divided into observation group and control group , two groups of patients were treated by intravenous chemotherapy of CE regimen , the observation group was given the local chemotherapy by fiber bronchoscope on the basis of conventional chemotherapy .Using Olympus BF-1T40 type of bronchoscopy , by bronchoscopy biopsy hole plastic thin catheter , injection of carboplatin 400mg spraying to the tumor surface; the control group was treated with intravenous chemotherapy .The two groups were observed clinical effect and Zubrod-ECOG-WHO performance status ( ZPS ) , Karnofsky performance status score ( KPS ) changes after one course of chemotherapy.Result:After treatment, ZPS, KPS scores inThe observation group were significant-ly higher than those in the control group ( P<0.05); the total effective rate the patients in the observation group was significantly higher than that of the control group ( P<0.05 ) , and the course of treatment side effects of mild poisoning .Conclusion: Local chemotherapy by fibrobronchoscopy can significantly improve the clinical symptoms of patients with central type lung cancer , decrease the tumor size significantly , It's ef-fective on middle and advanced stage central type lung cancer .
