目的总结α-氰基丙烯酸异丁酯眶前路直视下瘤内注射辅助眼眶血管性肿瘤摘除的手术治疗效果.方法临床病例系列研究.对2008年8月至2012年8月解放军474医院诊治了8例眼眶脉管性肿瘤,其中静脉畸形5例、动静脉畸形2例、淋巴管畸形1例.手术中暴露肿瘤前极,选择粗大血管粗针快速注入α-氰基丙烯酸异丁酯后进行肿瘤摘除,术后随访3月至2年,对其疗效进行评价.结果 8例血管性肿瘤较完整摘除,术中出血少,简化了操作.1例术后发生眶周局限性感染,经过抗感染治愈.所有病人术后随访期内外观改善满意,未见复发.结论 α-氰基丙烯酸异丁酯瘤内注射凝固后使脉管性肿瘤塑形,肿瘤边界更清楚,易与周围组织分离,术中的出血少,避免了肿瘤残余,可较完整摘除,简化了手术操作,是治疗眼眶脉管性肿瘤的有效方法.

Objective To summarize the surgical removal of an orbital vascular tumor assisted by anterior surgery intratumoral injection of alpha isobutyl cyanoacrylate.Methods Eight cases with orbital vascular tumors diagnosed and treated in the 474 Hospital of PLA from August 2008 to August 2012.Five cases with venous malformations,two cases with arteriovenous malformations,and one case with lymphatic malformation were found.During the operation,the tumor anterior pole was exposed,and alpha isobutyl cyanoacrylate was rapidly injected using a thick vascular needle.After injecting the glue,the tumor was removed.The patients were monitored for three months to two years after surgery,and the effect of operative treatment was evaluated.Results In the eight cases with orbital vascular tumors,the tumors were almost completely removed,The technical reduced the bleeding during the operation and simplifying the procedure.One case had a postoperative short-term orbital-localized infection,which eventually healed after anti-infection treatment.The improvements in the patients' appearances were satisfactory during the follow-up period,and no recurrence was observed.Conclusions Removal of orbital vascular tumor with assistance from anterior surgery embolization method can reduce bleeding during operation.The vascular tumor is shaped and the tumor boundary becomes more evident.Moreover,the tumor become easy to separate from the surrounding tissue after alpha isobutyl cyanoacrylate solidified.This effect can reduce residual tumors.The tumor removal becomes more complete,and the operation is simplified.The proposed method is effective in treating orbital vascular tumors.
