目的：探讨单孔后腹腔镜在肾囊肿去顶减压术中的应用价值。方法采用单孔后腹腔镜下完成肾囊肿去顶减压术12例，男5例，女7例。年龄40～71岁，平均52岁。左侧8例，右侧4例。囊肿直径4．5～11．5 cm，平均6．2 cm。术中先以气囊扩张建立后腹膜腔，然后置入自制单孔多通道装置，使用常规腹腔镜器械完成手术。结果12例手术均顺利完成，无中转开放手术或增加套管。手术时间45～120 min，平均62 min。术后引流管1～3 d拔出；术后住院时间4～7 d，平均5．5 d；术后随访1～14个月，经B超或CT复查，肾囊肿无复发。结论单孔后腹腔镜肾囊肿去顶减压术安全可靠，临床可行。熟练掌握标准腹腔镜技术，选择合适的病人是手术成功的关键。自制单孔多通道装置简单实用，值得在临床中推广使用。

목적：탐토단공후복강경재신낭종거정감압술중적응용개치。방법채용단공후복강경하완성신낭종거정감압술12례，남5례，녀7례。년령40～71세，평균52세。좌측8례，우측4례。낭종직경4．5～11．5 cm，평균6．2 cm。술중선이기낭확장건립후복막강，연후치입자제단공다통도장치，사용상규복강경기계완성수술。결과12례수술균순리완성，무중전개방수술혹증가투관。수술시간45～120 min，평균62 min。술후인류관1～3 d발출；술후주원시간4～7 d，평균5．5 d；술후수방1～14개월，경B초혹CT복사，신낭종무복발。결론단공후복강경신낭종거정감압술안전가고，림상가행。숙련장악표준복강경기술，선택합괄적병인시수술성공적관건。자제단공다통도장치간단실용，치득재림상중추엄사용。
Objective To evaluate the clinical value of retroperitoneal laparoscopic single-site surgery for the treatment of renal cysts. Methods 12 patients with renal cyst were treated with retroperitoneal laparoscopic single-site surgery,of which 5 patients were male and 7 female,aged from 40 to 71 years,and the average age was 52 years.The cysts were on the left side in 8 cases and on the right in 4 cases.Renal cysts were 4.5 to 11.5 cm in diameter,with an average diameter of 6.2 cm.The retroperitoneal cavity was set up with balloon dilatation,and the home-made single-port device was set in incision.All the operations were carried out using conventional lap-aroscopic instruments.Results The operation was completed successfully in all cases,with no conversion into open surgery or need for additional trocar.The operative time was 45 to 120 min,with an average time of62 min.The drainage tube was removed within 1 to 3 d.The postoperative hospitalization duration was 4 to 7 d,with an average time of 5 .5 d.In 1 2 patients,no recurrence of the cyst occurred during a follow-up of 1 to 1 4 months by B-ultrasonography or CT after operation.Conclusions Retroperitoneal lapa-roscopic single-site surgery for the treatment of renal cysts is reliable and feasible.Skillful laparoscopic operation and suitable indica-tions are the keys to successful surgery.The home-made single-port device is simple and practical,which worths being widely promo-ted in clinical.
