目的：探讨长时血压变异性（BPV）在脑白质疏松（LA）发病中的意义。方法高血压患者209例，每月随诊测血压1次，随访12个月，根据头颅MRI结果采用ARWMC评分法，比较LA组与非LA组以及LA组内不同程度白质疏松患者之间的长时血压变异性指标。结果 LA组长时平均收缩压（135.8±20.1）mmHg、平均舒张压（79.1±15.4）mmHg、收缩压标准差（13.0±2.7）mmHg、舒张压标准差（9.7±2.0）、收缩压变异系数（9.1±2.1）和舒张压变异系数（11.9±1.5），均高于非LA组的长时平均收缩压（125.4±17.1）mmHg、平均舒张压（74.5±7.9）mmHg、收缩压标准差（9.7±2.2）mmHg、舒张压标准差（7.8±1.0）mmHg、收缩压变异系数（7.0±1.9）和舒张压变异系数（10.5±1.3）（P均<0.05）。不同LA程度患者之间比较，长时平均血压、血压标准差及血压变异性差异无统计学意义（P>0.05）。结论长时血压变异性增大可影响LA的发生。
목적：탐토장시혈압변이성（BPV）재뇌백질소송（LA）발병중적의의。방법고혈압환자209례，매월수진측혈압1차，수방12개월，근거두로MRI결과채용ARWMC평분법，비교LA조여비LA조이급LA조내불동정도백질소송환자지간적장시혈압변이성지표。결과 LA조장시평균수축압（135.8±20.1）mmHg、평균서장압（79.1±15.4）mmHg、수축압표준차（13.0±2.7）mmHg、서장압표준차（9.7±2.0）、수축압변이계수（9.1±2.1）화서장압변이계수（11.9±1.5），균고우비LA조적장시평균수축압（125.4±17.1）mmHg、평균서장압（74.5±7.9）mmHg、수축압표준차（9.7±2.2）mmHg、서장압표준차（7.8±1.0）mmHg、수축압변이계수（7.0±1.9）화서장압변이계수（10.5±1.3）（P균<0.05）。불동LA정도환자지간비교，장시평균혈압、혈압표준차급혈압변이성차이무통계학의의（P>0.05）。결론장시혈압변이성증대가영향LA적발생。
Objective To investigate the relationship between long-term blood pressure variability (BPV) and leukoaraiosis (LA). Methods Two hundred and nine patients were consecutively recruited and followed up for 12 months. Blood pressure measurements were taken once per month at every interview. The patients were divided ac-cording to ARWMC scale evaluated from cranial MRI, and their long -term blood pressure and the index of long-term BPV were compared among LA group, non-LA group and every level group in LA group. Results In LA group, the systolic blood pressure(SBP) was 135.8±20.1 mmHg, the diastolic blood pressure(DBP) was 79.1±15.4 mmHg, the standard deviation of SBP was 13.0±2.7, the standard deviation of DBP was 9.7±2.0, and the coeffi-cient of variation of SBP and DBP were 9.1±2.1 and 11.9±1.5, which were higher than those of non-LA group (SBP: 125.4±17.1mmHg, DBP: 74.5±7.9mmHg, the standard deviation of SBP: 9.7±2.2, the standard deviation of DBP: 7.8±1.0, the coefficient of variation of SBP and DBP: 7.0±1.9 and 10.5±1.3. All P<0.05). No statistical dif ference was found among patients with different severity of LA (P>0.05). Conclusion This study indicates that long-term BPV affect the incidence of leukoaraiosis.
