目的：评估口服替吉奥联合三维适形放疗治疗局部晚期胃癌的疗效和安全性。方法：38例局部晚期胃癌患者进入研究，采用口服替吉奥联合同步三维适形放疗方案治疗，替吉奥在放疗开始的第1天口服，剂量为40-60mg/m2，2次/天，连服2周，休息1周。维持替吉奥口服治疗至放疗结束后4周，评价治疗疗效和毒副反应。结果：全部38例患者均完成同步放化疗，其中完全缓解3例，部分缓解26例，稳定4例，疾病进展5例，总有效率为76.3%；患者进食梗阻、疼痛主观症状改善率分别为78.8%、84.6%。全组1年总生存率55.3%，2年总生存率31.5%。主要不良反应是I-III度消化道反应及血液学毒性，经对症支持治疗后好转。结论：口服替吉奥联合三维适形放疗方案治疗局部晚期胃癌可以提高患者生活质量，不良反应可耐受。
목적：평고구복체길오연합삼유괄형방료치료국부만기위암적료효화안전성。방법：38례국부만기위암환자진입연구，채용구복체길오연합동보삼유괄형방료방안치료，체길오재방료개시적제1천구복，제량위40-60mg/m2，2차/천，련복2주，휴식1주。유지체길오구복치료지방료결속후4주，평개치료료효화독부반응。결과：전부38례환자균완성동보방화료，기중완전완해3례，부분완해26례，은정4례，질병진전5례，총유효솔위76.3%；환자진식경조、동통주관증상개선솔분별위78.8%、84.6%。전조1년총생존솔55.3%，2년총생존솔31.5%。주요불량반응시I-III도소화도반응급혈액학독성，경대증지지치료후호전。결론：구복체길오연합삼유괄형방료방안치료국부만기위암가이제고환자생활질량，불량반응가내수。
Objective:To evaluate the efficacy and toxicity of S-1 combined with three dimensional conformal ra-diotherapy for patients with locally advanced gastric cancer. Methods:Thirty eight patients with locally advanced gas-tric cancer enrolled in this study. Patients received three dimensional conformal radiotherapy,the radiation dose was 60Gy in daily 2. 0Gy fractions,5 times weekly. At the same time,the patients took S-1 40-60mg/m2 ,twice per day for two weeks,followed by one week rest. Results:The therapeutic response was evaluated in 38 patients(3 complete response,26 partial response,4 stable disease and 5 progrssive disease)with response rate of 76. 3%. The remission rate of obstruction,bleeding were 78. 8%,84. 6%. The 1-,2-year survival rates were 55. 3%、31. 5%. The major adverse effects were vomiting and neuraltoxicity of I-III grade which took a favorable turn after heteropathy. Conclu/sion:Oral S-1 regimen combined with three dimensional conformal radiotherapy shows good quality of life and tolera-ble toxicity in the treatment of patients with locally advanced gastric cancer.
