目的：探讨西维来司钠对家兔早期肺爆震伤后血浆肿瘤坏死因子-α（ TNF-α）、白细胞介素-6（IL-6）、白细胞介素-8（IL-8）的影响。方法将30只健康新西兰大白兔随机分为2组：对照组（A组），15只，治疗组[B组，西维来司钠（Sweden，CAS 150374-951）]，15只。利用雷管爆炸制作兔肺爆震伤动物模型。治疗组伤后立即给予西维来司钠10mg／kg耳缘静脉注射；对照组经耳缘静脉注射同等剂量的生理盐水。用双抗夹心酶联免疫分析法测定治疗前及伤后1、6、12、24h血浆中TNF-α、IL-6、IL-8含量的变化。解剖家兔肺组织，制作石蜡切片，HE染色后观察病理变化。结果应用西维来司钠后血浆中TNF-α、IL-6、IL-8的水平在1、6、12 h三点较对照组明显下降，肺大体标本及组织病理学水肿、出血表现较对照组也明显减轻。结论西维来司钠能够降低肺爆震伤后血浆中TNF-α、IL-6、IL-8的含量，有效抑制损伤导致的炎症反应，从而减轻早期肺爆震伤的损害。
목적：탐토서유래사납대가토조기폐폭진상후혈장종류배사인자-α（ TNF-α）、백세포개소-6（IL-6）、백세포개소-8（IL-8）적영향。방법장30지건강신서란대백토수궤분위2조：대조조（A조），15지，치료조[B조，서유래사납（Sweden，CAS 150374-951）]，15지。이용뢰관폭작제작토폐폭진상동물모형。치료조상후립즉급여서유래사납10mg／kg이연정맥주사；대조조경이연정맥주사동등제량적생리염수。용쌍항협심매련면역분석법측정치료전급상후1、6、12、24h혈장중TNF-α、IL-6、IL-8함량적변화。해부가토폐조직，제작석사절편，HE염색후관찰병리변화。결과응용서유래사납후혈장중TNF-α、IL-6、IL-8적수평재1、6、12 h삼점교대조조명현하강，폐대체표본급조직병이학수종、출혈표현교대조조야명현감경。결론서유래사납능구강저폐폭진상후혈장중TNF-α、IL-6、IL-8적함량，유효억제손상도치적염증반응，종이감경조기폐폭진상적손해。
Objective To investigate the effect of sivelestat sodium on plasma TNF-α,IL-6,IL-8 in rabbits after early lung injury.Methods Totally 30 healthy New Zealand rabbits were randomly divided into 2 groups:group A was the control group with 15 rabbits;B group was the treatment group ( sivelestat sodium, Sweden, CAS 150374-95-1) with 15 rabbits.Detonator explosion was employed to make rabbit blast lung injury models.Siveles-tat sodium(10mg/kg) was given through ear vein injection immediately after injury in group B;saline of the same dose was given through intravenous injection in group A.The content changes of TNF-α,IL-6,IL-8 at the time pha-ses of 1,6,12h and 24h after injury in plasma were measured by ELISA.Lung tissue of the rabbits was dissected and paraffin section was made.The pathological changes were observed after HE staining.Results The levels of TNF-α,IL-6,IL-8 in plasma in group B were significantly decreased compared with group A after treatment at time phases of 1,6 and 12h.The condition of edema and hemorrhage of lung tissue in group B was significantly relieved compared with group A.Conclusion Sivelestat sodium can reduce the content of TNF-α,IL-6,IL-8 in plasma after lung blast injury and inhibit consequent inflammatory reaction,thereby reducing damage of blast injury to the lung.
