目的评价S形钛网眶底植入治疗眶下壁骨折后眼球内陷的效果.方法回顾性分析17例(17只眼)眶下壁骨折眼球内陷患者行S形钛网眶底植入手术的临床资料.术前眼球内陷2～3 mm者9例,眼球内陷3.5 ～4.5 mm者8例,复视3例,眼球向下移位2例.术中均经下睑缘睫毛下皮肤切口,骨膜下植入塑形钛网覆盖眶下壁骨折缺损区.结果术后随访6 ～ 12个月,眼球内陷均矫正至2 mm以内,复视3例完全消失,2例眼球下移位均消失,视力无明显变化.术后无感染,植人物无脱出、排斥或移位等并发症,术后出现1例轻度下睑退缩.结论单纯应用S形钛金属眶底植入治疗眶下壁骨折引起的眼球内陷安全有效.
목적 평개S형태망광저식입치료광하벽골절후안구내함적효과.방법 회고성분석17례(17지안)광하벽골절안구내함환자행S형태망광저식입수술적림상자료.술전안구내함2～3 mm자9례,안구내함3.5 ～4.5 mm자8례,복시3례,안구향하이위2례.술중균경하검연첩모하피부절구,골막하식입소형태망복개광하벽골절결손구.결과 술후수방6 ～ 12개월,안구내함균교정지2 mm이내,복시3례완전소실,2례안구하이위균소실,시력무명현변화.술후무감염,식인물무탈출、배척혹이위등병발증,술후출현1례경도하검퇴축.결론 단순응용S형태금속광저식입치료광하벽골절인기적안구내함안전유효.
Objective To evaluate the effect of "S" shaped titanium mesh implantation on orbit floor for enophthalmos secondary to inferior orbital wall fracture.Methods The surgical effects of 17 eyes of 17 cases inferior orbital wall fracture who received "S" shaped titanium mesh implantation on orbit floor for orbital wall reconstruction were analyzed retrospectively.Preoperatively,there were 9 cases with enophthalmos between 2 to 3 mm,8 cases with enophthalmos between 3.5 to 4.5 mm,3 cases with diplopia,2 cases with apparent vertical globe dystopia.For all the cases,an inferior subciliary approach was performed,a "S" shaped titanium mesh plate was implanted under the periosteum to cover the area of the inferior orbital wall fracture for reconstruction of the orbital cavity.Results Postoperatively,on the follow-up period of 6-12 months,the enophthalmos of all cases was corrected to less than 2mm successfully,the preoperative diplopia disappeared in all the 3 cases,the vertical globe dystopia of 2 eases disappeared,and no visual complication was observed.No complication of infection,extrusion,rejection or migration of the implants occurred,however mild low eyelid retraction occurred in 1 case and disappeared at 8 month postoperatively.Conclusion "S" shaped titanium mesh plate implantation in orbital reconstruction for correcting enophthalmos secondary to inferior orbital wall fracture is safe and effective.
