Laser in situ keratomileusis(LASIK)%Dynamic cyclotorsion
目的 观察准分子激光原位角膜磨镶术(LASIK)中眼球动态旋转的方向和角度.方法 前瞻性系列病例研究,对近视及近视性散光133例(235只眼)应用阿玛仕准分子激光仪行LASIK手术,按照眼别、术前屈光度分组后,观察术中眼球动态旋转的方向和角度.计量资料采用独立样本t检验进行分析.结果 动态旋转角度范围(绝对值)0°~7.88°.右眼动态旋转角度范围(绝对值)0°~7.77°,平均为1.74°±1.46°;左眼动态旋转角度范围(绝对值)0°~7.88°,平均为1.90°±1.54°,右眼与左眼比较差异无统计学意义(t=0.69,P=0.25).133例(235只眼)均测出了动态旋转:内旋者26只眼(11.06%),旋转角度0°~7.88°,平均为2.03°±2.05°;外旋者12只眼(5.11%),旋转角度0.18°~ 6.81°,平均为2.16°±1.87°;混合性旋转197只眼(83.83%),旋转角度0.01°~5.79°,平均为1.74°±1.34°.全部235眼动态旋转幅度为0.01°~10.5°,平均为3.54°±2.00°;右眼平均幅度为3.26°±1.65°,左眼平均幅度为3.33°±1.71°,两者比较,差异无统计学意义(t=0.93,P =0.20).动态旋转幅度:术前屈光度高度组>屈光度中度组>屈光度低度组,且3组均值比较差异有统计学意义(t=4.10,P=0.001;t=3.04,P=0.002;t =6.06,P=0.001).结论 LASIK术中眼球均发生动态旋转,近视度愈高,旋转角度和总幅度愈大,因此需要实时测量动态旋转并调整激光发射位置进行补偿.

Objective To evaluate the direction and degree of dynamic cyclotorsion during laser in situ keratomileusis(LASIK).Methods This retrospective study comprised 235 eyes of 133 patients that had LASIK for myopia with or without astigmatism by Schwind Amaris excimer laser.According to eyes,preoperative diopter of spherical power were divided into groups,dynamic cyclotorsion was assessed during LASIK.Enumeration data were compared with independent samples test.Results Dynamic cyclotorsion degree range from 0° to 7.88° (absolute value).Dynamic cyclotorsion degree of right eyes range from 0° to 7.77° (absolute value)the mean AP was 1.74° ± 1.46°.Dynamic cyclotorsion degree of left eyes range from 0° to 7.88° (absolute value) the mean AP was 1.90° ± 1.54°.There was no significant statistical difference between right eyes and left eyes (t =0.69,P =0.25).235 eyes were successful measured.26 eyes (11.06%) had incyclotorsion,range from 0 degrees to 7.88 degrees,the mean AP was 2.03 ° ± 2.05 °;12 eyes (5.11%) had excyclotorsion,range from 0.18 degrees to 6.81 degrees,the mean AP was 2.16° ± 1.87°;197 eyes(83.83%) had both excyclotorsion and incyclotorsion,range from 0.01 degrees to 5.79 degrees.the mean AP was 1.74° ± 1.34 °.The dynamic cyclotorsion range from 0.01 degrees to 2.24 degrees,the mean AP was 3.54° ± 2.00°.The dynamic cyclotorsion amplitude of right eyes was 3.26° ± 1.65°,left eyes was 3.33° ± 1.71 °.There was no significant statistical difference between right eyes and left eyes(t =0.93,P =0.20).Cyclotorsion amplitude had been compared:high-diopter > mid-diopter > low-diopter.There were statistical differences between three groups(t =4.10,P =0.001;t =3.04,P =0.002;t =6.06,P =0.001).Conclusion Dynamic cyclotorsion occurs during LASIK.The higher myopic degree is,the larger the eye cyclotorsion degree and the total amplitude will be.So Dynamic cyclotorsion should be measured and compensated.
