目的:通过胎儿生长受限新生儿及正常新生儿不同分娩方式下新生儿脐血血气分析,研究胎儿生长受限新生儿脐血血气分析特点及临床意义。方法将2013年我院确诊为胎儿生长受限的孕产妇,顺产的28例为 A 组,随机选取行择期剖宫产术的30例为 B 组,随机选取同期适龄胎儿顺产及行择期剖宫产产妇各30例为 C 组、D 组。分别对比各组新生儿的脐动脉血 pH、PO2、PCO2、BE、HCO3水平。结果 A 组的脐动脉 pH、BE、HCO3均低于 B 组(P<0.05),差异有显著性。A 组的脐动脉 pH、BE、HCO3均低于 C 组(P <0.05),差异有显著性。B 组的脐动脉 BE 低于 D 组(P <0.05),B 组 PCO2高于 D 组(P <0.05),差异有显著性。C 组的脐动脉 BE 低于 D 组(P <0.05),B 组PCO2高于 D 组(P <0.05),差异有显著性。结论阴道分娩较剖宫产对新生儿脐血影响较大,胎儿生长受限胎儿存在着缺氧状态,对阴道分娩耐受性差,适当放宽剖宫产指征,有利于降低其缺氧及并发症的发生,改善胎儿生长受限新生儿的预后。

목적:통과태인생장수한신생인급정상신생인불동분면방식하신생인제혈혈기분석,연구태인생장수한신생인제혈혈기분석특점급림상의의。방법장2013년아원학진위태인생장수한적잉산부,순산적28례위 A 조,수궤선취행택기부궁산술적30례위 B 조,수궤선취동기괄령태인순산급행택기부궁산산부각30례위 C 조、D 조。분별대비각조신생인적제동맥혈 pH、PO2、PCO2、BE、HCO3수평。결과 A 조적제동맥 pH、BE、HCO3균저우 B 조(P<0.05),차이유현저성。A 조적제동맥 pH、BE、HCO3균저우 C 조(P <0.05),차이유현저성。B 조적제동맥 BE 저우 D 조(P <0.05),B 조 PCO2고우 D 조(P <0.05),차이유현저성。C 조적제동맥 BE 저우 D 조(P <0.05),B 조PCO2고우 D 조(P <0.05),차이유현저성。결론음도분면교부궁산대신생인제혈영향교대,태인생장수한태인존재착결양상태,대음도분면내수성차,괄당방관부궁산지정,유리우강저기결양급병발증적발생,개선태인생장수한신생인적예후。
Objective Study the blood gases of cord blood about FGR neonate and clinical value through the blood gases of cord blood about neonate between natural labour and caesarean birth.Methods A group are the FGR neonate who are natural labour;B group are the FGR neonate who are caesarean birth.C group are the nomal weight neonate who are caesarean birth.D group are the nomal weoght neonate who are caesarean birth.We compare the difference of pH,PO2 ,PCO2 ,BE,HCO3 between each group.Results pH,BE,HCO3 in group A are lower than that in group B. pH,BE,HCO3 in group A are lower than that in group C.BE in group B was lower,while PCO2 was higher in group D. BE in group C was lower,while PCO2 was higher in group D.Conclusions natural labour was more influence on the blood gases of cord blood than caesarean birth.The FGR neonates.are aerobic condition.Their abillity to tolerate the natural labour are lower than normal weight neonates.The roperly relaxation of caesarean birthindication can lower the incidence of anoxia neonatorum,and improve the prognosis of FGR neonate.
