目的:通过分析肝硬化合并肝肾综合征(HRS)患者外周血肾损伤因子-1(KIM-1)水平,探讨 KIM-1的早期诊断、预测价值。方法选择肝硬化患者60例(单纯肝硬化患者35例,肝硬化合并 HRS 患者25例),选择健康体检者30例作对照,比较不同组患者 Scr、BUN、eGRF、KIM-1和血清 Cys-C 水平,分析 KIM-1诊断 HRS 的 ROC 曲线以及 KIM-1水平与 Cys-C、Scr 和 eGRF 的相关性。结果随着 Child-Pugh 分级的上升,患者 Scr、BUN、KIM-1和Cys-C 水平增高,eGRF 水平降低,差异有统计学意义(P <0.05)。合并 HRS 组患者 KIM-1和 Cys-C 水平均高于单纯肝硬化组,差异有统计学意义(P <0.001)。KIM-1诊断 HRS 的 AUC 为0.808(P <0.05)。肝硬化患者血清 KIM-1水平与 Cys-C、Scr 呈正相关;与 eGRF 呈负相关。结论外周血 KIM-1水平能够比较灵敏地反映肝硬化合并肝肾综合征患者的早期肾损害,对其早期诊断、预测有重要的临床价值。

목적:통과분석간경화합병간신종합정(HRS)환자외주혈신손상인자-1(KIM-1)수평,탐토 KIM-1적조기진단、예측개치。방법선택간경화환자60례(단순간경화환자35례,간경화합병 HRS 환자25례),선택건강체검자30례작대조,비교불동조환자 Scr、BUN、eGRF、KIM-1화혈청 Cys-C 수평,분석 KIM-1진단 HRS 적 ROC 곡선이급 KIM-1수평여 Cys-C、Scr 화 eGRF 적상관성。결과수착 Child-Pugh 분급적상승,환자 Scr、BUN、KIM-1화Cys-C 수평증고,eGRF 수평강저,차이유통계학의의(P <0.05)。합병 HRS 조환자 KIM-1화 Cys-C 수평균고우단순간경화조,차이유통계학의의(P <0.001)。KIM-1진단 HRS 적 AUC 위0.808(P <0.05)。간경화환자혈청 KIM-1수평여 Cys-C、Scr 정정상관;여 eGRF 정부상관。결론외주혈 KIM-1수평능구비교령민지반영간경화합병간신종합정환자적조기신손해,대기조기진단、예측유중요적림상개치。
Objective By analyzing the peripheral kidney injury molecule-1 levels of patients with hepatorenal syn-drome,to explore the early diagnosis and predictive value of KIM-1.Methods 60 patients of liver cirrhosis (35 patients with cirrhosis alone,25 patients with liver cirrhosis complicated HRS)were selected,and 30 cases of healthy people were selected as control group,Scr,BUN,eGRF,KIM-1 and Cys-C levels were compared in different groups,the ROC curve of KIM-1 diagnosis on HRS patients,correlation analysis of KIM-1 levels and Cys-C,Scr and eGRF were conduc-ted.Results With the rise of the Child-Pugh classification,Scr,BUN,KIM-1 and Cys-C levels increased,and eGRF lev-el reduced,the difference was statistically significant (P <0.05).The KIM-1 and Cys-C levels in combined HRS group were higher than those in cirrhosis group,the difference was statistically significant (P <0.001).The AUC of KIM-1 diagnosis on HRS was 0.808 (P <0.05).Serum KIM-1 and Cys-C,Scr were positively correlated;KIM-1 and eGRF were negative correlated.Conclusion Peripheral blood KIM-1 level is a sensitive response indicator of early renal dam-age in patients with hepatorenal syndrome,and it has important clinical value for early diagnosis and prediction.
