目的:探讨罗哌卡因行肋间神经阻滞对胸部手术患者术后的镇痛效果。方法:拟行胸部手术患者60例,随机均分为两组。罗哌卡因组:肋间神经应用0.375%罗哌卡因35 mL阻滞,对照组:0.9%生理盐水35 mL。观察两组患者术后4 h、8 h、12 h、24 h、48 h疼痛VAS评分及Prince-Henry(P-H)疼痛评分。并观察两组患者术后不良反应。结果:术后罗哌卡因组VAS及Prince-Henry评分均明显低于对照组(P<0.01),罗哌卡因组呼吸系统并发症明显少于对照组(P<0.01)。结论:0.375%罗哌卡因肋间神经阻滞可有效减轻开胸术后胸部疼痛并减少呼吸系统并发症。

목적:탐토라고잡인행륵간신경조체대흉부수술환자술후적진통효과。방법:의행흉부수술환자60례,수궤균분위량조。라고잡인조:륵간신경응용0.375%라고잡인35 mL조체,대조조:0.9%생리염수35 mL。관찰량조환자술후4 h、8 h、12 h、24 h、48 h동통VAS평분급Prince-Henry(P-H)동통평분。병관찰량조환자술후불량반응。결과:술후라고잡인조VAS급Prince-Henry평분균명현저우대조조(P<0.01),라고잡인조호흡계통병발증명현소우대조조(P<0.01)。결론:0.375%라고잡인륵간신경조체가유효감경개흉술후흉부동통병감소호흡계통병발증。
Objective:To explore the effect of ropivacaine intercostal nerve block for the postoperative analgesia in the patients with chest operation.Methods:60 patients with chest operation were randomly divided into two groups.The ropivacaine group was given 0.375% ropivacaine 35 mL to block intercostal nerve.The control group was given 0.9% physiological saline 35 mL.The postoperative 4 hours,8 hours,12 hours,24 hours,48 hours VAS pain scores and prince-Henry pain scores(P-H) of patients in two groups were observed.The postoperative adverse reactions of patients in two groups were observed.Results:The postoperative VAS and prince-Henry pain scores of the ropivacaine group were significantly lower than those of the control group(P<0.01).The respiratory system complications of the ropivacaine group was significantly lower than that of the control group(P<0.01). Conclusion:0.375% ropivacaine intercostal nerve block can effectively reduce open chest postoperative chest pain and reduce the complications of respiratory system.
