目的：观察单侧组合式外固定架治疗高龄股骨粗隆间骨折合并基础疾病的临床疗效。方法对27例高龄股骨粗隆间骨折合并基础疾病的患者采用闭合复位单侧组合式外固定架固定治疗，观察其疗效。结果手术时间为30～60 min，平均40 min；术中出血量为20～40 mL，平均23．33 mL。有5例患者发生针道浅表感染，3例患者因过早负重致轻度髋内翻畸形。骨折平均愈合时间为4个月，最短2．5个月。髋关节疗效为优5例，良19例，可3例，优良率为88．89％。结论闭合复位单侧组合式外固定架治疗股骨粗隆间骨折，具有手术安全性高、操作损伤小、技术要求简单、易于推广、避免二次手术及患者与家属容易接受等优点，尤其适用于不能耐受内固定手术的高龄合并有基础疾病的体弱患者。

목적：관찰단측조합식외고정가치료고령고골조륭간골절합병기출질병적림상료효。방법대27례고령고골조륭간골절합병기출질병적환자채용폐합복위단측조합식외고정가고정치료，관찰기료효。결과수술시간위30～60 min，평균40 min；술중출혈량위20～40 mL，평균23．33 mL。유5례환자발생침도천표감염，3례환자인과조부중치경도관내번기형。골절평균유합시간위4개월，최단2．5개월。관관절료효위우5례，량19례，가3례，우량솔위88．89％。결론폐합복위단측조합식외고정가치료고골조륭간골절，구유수술안전성고、조작손상소、기술요구간단、역우추엄、피면이차수술급환자여가속용역접수등우점，우기괄용우불능내수내고정수술적고령합병유기출질병적체약환자。
Objective To observe the clinical effect of unilateral hybrid external fixator on in-tertrochanteric femoral fractures in elderly patients with underlying diseases.Methods Twenty-seven elderly patients with intertrochanteric femoral fractures complicated by underlying diseases were treated with closed reduction and unilateral hybrid external fixation.Results The average operation time was 40 minutes (range,30-60 minutes),and the average intraoperatve blood loss was 23.33 mL (range,20-40 mL).Superficial pin tract infection occurred in 5 patients,and prema-ture loading-induced mild hip varus deformity occurred in 3 patients.The average healing time was 4 months and the shortest healing time was 2.5 months.Among the 27 patients,hip function was excellent in 5,good in 19,and fair in 3.The excellent and good rate was 88.89%.Conclusion The treatment with closed reduction and unilateral hybrid external fixation for intertrochanteric femoral fractures has the advantages of high safety,small damage,simple operation,facile popu-larization,avoidance of second surgery,and easy acceptance by patients and their families,espe-cially for elderly patients with underlying diseases who can not tolerate internal fixation.
